Objective: By comparing and analyzing the characteristics of gender composition,family history,Helicobacter pylori infection,tumor markers,endoscopic classification,initial symptoms,CEA,CA199,AFP,pathological classification and TNM staging between young gastric cancer and elderly gastric cancer,we can provide a direction for early clinical diagnosis and treatment.Methods: 1168 cases of gastric cancer from May 2015 to June 2020,admitted by the First People’s Hospital of Yueyang and Yueyang Second People’s Hospital,were collected with all patients needed gastroscopy,biopsy or surgery to make pathological diagnosis clear,and eliminating other malignant tumors,gastric stump cancer and incomplete medical records.According to their age,they are divided into the young group and the elderly group(44 years old is as the boundary between them).This study mainly analyzes retrospectively: gender composition,family history,Helicobacter pylori infection,tumor markers,endoscopic classification,initial symptoms,tumor location,pathological classification and TNM staging,etc.Results:1.The proportion of women in the young group(55.6%)is significantly higher than that in the middle-aged group(29.6%),and the difference is statistically significant(P < 0.05).2.The proportion of young group with family history(12.5%)is higher than that in elderly group(2.6%),the difference is statistically significant(P < 0.05).3.The proportion of Hp infection in young group is higher than that in elderly group,and the difference is statistically significant(P < 0.05).4.There is no significant difference in serum CEA,CA199 and AFP levels between the two groups(P> 0.05).5.Borrmann type II and III are the main types in the young group and the elderly group.Type IV is more common in the young group than in the elderly group,and the differences are significant(P<0.05).6.Abdominal distention or abdominal pain is the main manifestation of the two groups,and the proportion of dysphagia in the young group is significantly less than that in the elderly group,with significant differences(P<0.05).7.Gastric antrum and body are the main tumor sites in the young group and the elderly group,but the proportion of cardia and fundus in the young group is less than that in the elderly group,and the difference is statistically significant(P< 0.05).8.The proportion of poorly differentiated adenocarcinoma and signet ring cell carcinoma in young group is significantly higher than that in elderly group,but the proportion of moderately differentiated adenocarcinoma is significantly higher than that in young group,and the proportion of poorly differentiated adenocarcinoma and signet ring cell carcinoma in young group is higher than that in elderly group,and the differences are statistically significant(P< 0.05).9.TNM stage: the young group is mainly stage III and IV,and the proportion of stage IV in the young group is higher than that in the elderly group,with significant difference,and the stage is later.Conclusion:1.In young gastric cancer patients,the proportion of female patients is slightly more than that of male patients,while the proportion of male patients is significantly more than that of female patients in elderly groups;young gastric cancer patients have more VII obvious family background factors;the proportion of Helicobacter pylori infection in young gastric cancer patients is higher.2.Gastric cancer in young people,abdominal pain in elderly people and abdominal distension are common clinical manifestations.Dysphagia in young people is less than that in elderly people;gastric cardia and fundus in young people are less than that in elderly people,and the proportion of Borrmann type IV is higher than that in elderly people.3.Poorly differentiated adenocarcinoma and signet ring cell carcinoma are common in young patients with gastric cancer.4.The young patients with gastric cancer have a late stage,mainly in stage III and IV.The proportion of stage IV is higher than that of the elderly patients.
