| Objective:To investigate the relationship between plasma hs-cTnT,H-FABP,copeptin,GDF-15 and GRACE score in patients with non ST segment elevation myocardial infarction(NSTEMI).It is expected that the above indicators can be used in the risk stratification of GRACE score and have clinical value for early risk assessment and prognosis of NSTEMI patients.Methods:97 patients with NSTEMI admitted to the Affiliated Hospital of North Sichuan Medical College from January to December 2020 were selected and divided into high-risk,medium risk and low-risk groups by garce score.General clinical data and laboratory data were recorded.Venous blood was taken immediately after admission.The levels of H-FABP,copeptin and GDF-15 weredetermined by ELISA.If patients underwent coronary angiography,Gensini score was recorded.Spss21.0 software was used for data analysis,P<0.05 was statistically significant.Results:Single factor analysis,according to the results of each index comparison between different GRACE score group,age,Body mass index(BMI),diastolic blood pressure(DBP),the rate of abnormal electrocardiogram,Killip classification,creatinine,total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),NT-proBNP,myoglobin,hs-cTnT,H-FABP,copeptin,GDF-15,a total of 14 indexes between groups was statistically significant difference(P<0.05).Pairable comparison results showed that age,Killip grade,H-FABP,copeptin,BMI,creatinine,LDL-C,NT-proBNP,ECG abnormal rates in high-risk group were significantly higher than those in low-risk group;DBP in high risk group was significantly lower than that in medium risk group,and medium risk group was lower than that in low risk group(P<0.05).TC in high risk group was significantly lower than that in low risk group(P<0.05);There were no significant differences between smoking history and no smoking history,hypertension history and no hypertension history,diabetes history and no diabetes history.Logistic regression model showed that age(OR=1.410,P<0.05),DBP(OR=0.864,P<0.05),copeptin(OR=1.052,P<0.05),GDF-15(OR=1.015,P<0.05),hs-cTnT(OR=3.458,P<0.05)were significantly correlated with GRACE score.Age,copeptin,GDF-15,hs-cTnT(OR=3.458,P<0.05)were independent positive correlation factors,while diastolic blood pressure were independent negative correlation factors.Spearman rank correlation analysis showed that hs-cTnT,h-FABP,copeptin,GDF-15 were significantly positively correlated with GRACE scores(r>0,P<0.05).Conclusion:Hs-cTnT,h-FABP,copeptin,GDF-15 were significantly positively correlated with GRACE scores.We may consider using plasma h-FABP,copeptin,and GDF-15 levels as a supplement to risk stratification of patients with NSTEMI by GRACE score. |
