Objective:To investigate the clinical and pathological traits of synchronous endometrial and ovarian carcinoma(SEOC),and to analyze the relative factors affecting prognosis.To provide evidence that how to diagnose and remedy individually about this relatively rare gynecological tumor.Methods:A total of 130 SEOC patients who met the criteria for diagnosis were selected from Nation Cancer Institute&Hospital Chinese Academy of Medical Science from 1997 to 2019.The intact clinical medical recording document and follow-up data were required.Pathological diagnosis was reviewed by pathologists,and we set up a clinico-pathological database in order to statistical analysis.The clinical and pathological characteristics,diagnosis,therapy and prognosis of SEOC patients were retrospectively deliberated,and the related factors which made an impact on the prognosis of SEOC patients were adequately discussed.Results:Among the 130 SEOC patients,the mean age of diagnosis was 51 years old,the mean BMI was 24.6 kg/m2,the premenopausal patients accounted for 49.2%(64/130 cases),the patients with abnormal uterine bleeding first diagnosed accounted for 45.4%(59/130 cases),and the patients with increased CA125 accounted for 87.7%(114/130 cases).The main pathological types of endometrioid carcinoma were endometrioid carcinoma,accounting for 54.6%(71/130 cases).Stage I SEOC accounted for 41.5%(54/130 cases).The infiltration depth of endometrial cancer focus was dominated by no muscular invasion or only superficial muscular invasion,accounting for 80.3%(57/130 cases).In this study,all SEOC patients completed surgical treatment,among which 108 patients(83.1%)received lymph node dissection,123 patients(88.5%)received omentum resection,and 115 patients(88.5%)received postoperative adjuvant treatment.Survival analysis:Multi-factor analysis showed that the patients with positive ascites or positive pelvic and abdominal rinse fluid(P<0.01),pathological type of non-A SEOC(P<0.05).ovarian cancer with non-endometriosis(P<0.01),higher than stage Ⅰ SEOC(P<0.01)had A relatively poor prognosis,which were independent influencing factors of PFS in patients with SEOC.Patients with positive ascites or basin-peritoneal rinse fluid(P<0.05)and ovarian cancer with focal stage higher than I(P<0.05)had a relatively poor prognosis,which were independent influencing factors for OS in patients with SEOC.Conclusion:The onset age of SEOC is younger than that of endometrial cancer or ovarian cancer,and early patients are more likely to see it,and the prognosis is relatively good with pathology A-class SEOC.The stage of operation,the type of pathology,whether ascites or pelvis rinse fluid is positive or not are important factors affecting the prognosis of SEOC,and individualized treatment should be paid attention to after operation to avoid excessive treatment.
