| Objective:The main purpose of this study was to examine the two most commonly used materials:Autogenous costal cartilage and Medpor bracket, through multiple dimensions to compare the characteristics of both and provide reference data and relevant information of auricle reconstruction for the frontline plastic surgeons,so as to reduce the incidence of side effects as far as possible, and then improve the satisfaction and effect of treatment。Materials and Methods:Literature retrieval was conducted on PubMed,Cochrane,Library,Embase,CNKI and Wanfang Medical Spectator Database, and relevant literatures published from March 1,2008 to March 1,2021 were summarized and sorted.According to certain inclusion and exclusion criteria,Meta analysis was performed in combination with the literature that met the requirements.According to the clinical application of autogenous costal cartilage and MedPor scaffold material, the complications and satisfaction were compared. Results:Fourteen retrospective observational studies, including 4987 patients, published between 2008 and 2021 were included. The incidence of postoperative infection, cartilage exposure, hematoma, stent deformation and postoperative satisfaction rate of the stents in the selected literature were compared and analyzed.Among them, the incidence of cartilage exposure after the treatment of microtia with Medpor stent was significantly higher than that with autogenous rib cartilage(P=0.007),which was statistically significant. There was no significant difference in the complications of infection, hematoma and stent degeneration。Conclusion:Autogenous rib cartilage has the advantages of low rejection reaction, easy to take material, sufficient amount, easy to carve, flexibility, good histocompatibility,long-term stability, low infection rate, few complications, etc.,and satisfactory postoperative effect。It is an ideal material for auricle reconstruction, and should be the first choice if there is no contraindication。MedPOR ear stent has the advantages of easy assembly, good histocompatibility, easy vascular insertion, simple operation,time-saving operation, less trauma, ideal shape of reconstructed ear, etc. However, the incidence of stent exposure is high and there is a risk of surgical failure。Therefore, indications should be strictly controlled to prevent skin necrosis and stent exposure。Due to the lack of long-term follow-up of large samples, its efficacy remains to be further observed。... |
