| Objective:This project adopts a ward-outpatient-home full-course integrated nursing measure based on a full-course management model for patients with type 2 diabetic retinopathy,analyze the effects on fasting blood glucose,2 hours postprandial blood glucose,blood pressure,body,mass index,glycated hemoglobin,medication compliance,self-management ability and quality of life,to provide a theoretical basis for optimizing the comprehensive nursing management of patients with diabetic retinopathy.Method:This study is a non-contemporaneous controlled study.Patients admitted to the endocrinology department of a tertiary first-class hospital in Binzhou from April 2019 to June 2019 were selected as the control group.From September to November 2019,patients in the endocrinology department of a tertiary first-class hospital in Binzhou were the experimental group.Patients in the control group received routine ward care and routine continuous nursing measures after discharge.Patients in the experimental group implemented interventions based on the ward-outpatient-family comprehensive nursing model.Patients in the two groups were evaluated at the time of discharge and 3 months after discharge.The patients had fasting blood glucose,2 hours postprandial blood glucose,glycated hemoglobin,body mass index,blood pressure,medication compliance,self-management ability,and quality of life.Result:1.After 3 months of ward-outpatient-family full-course nursing mode intervention for patients,the measured values of FBG,2hPBG,and HbA1C were statistically different between the two groups(P<0.05).2.After 3 months of ward-outpatient-family-based comprehensive nursing mode intervention for patients,there was a statistically significant difference in the medication compliance between the two groups of patients(P<0.05).3.There was no statistical difference in visual acuity between the two groups of patients after 3 months of ward-outpatient-family full-course nursing mode intervention(P>0.05).4.Three months after the intervention of the ward-outpatient-family full-course nursing mode for patients,the total management ability score,the prevention and treatment dimensions of high and low blood glucose,the results were statistically different(P<0.05).5.After implementing the ward-outpatient-family comprehensive nursing mode intervention for patients,the two dimensions of total quality of life,satisfaction items and social and family-related anxiety items of the two groups of patients were statistically different.(P<0.05).Conclusion:1.The integrated ward-outpatient-home nursing based physical model can significantly improve FBG,2hPBG and HbA1C in patients with diabetic retinopathy.2.The integrated ward-outpatient-home care based physical model can significantly improve the medication compliance of patients with diabetic retinopathy.3.The integrated ward-outpatient-home nursing integrated physical measures based on the whole-process model can significantly improve patients’ self-management ability and quality of life. |
