Objective:to investigate the clinical effects of remote ischemic conditioning(RIC)combined with exercise rehabilitation(ER)on 6-minute walking distance(6MWD),NT-proBNP,and left ventricular ejection fraction(LVEF)、Left ventricular end systolic volume(LVESV)、Left ventricular end-diastolic volume(LVEDV)in patients with chronic heart failure(CHF),and to explore its long-term prognostic value in patients with chronic heart failure.Methods:A total of 100 patients who were admitted to the Department of Cardiology of Yantai Affiliated Hospital of Binzhou Medical College from August 2017 to August 2018 and met the diagnostic criteria for chronic heart failure were selected.All patients met the exclusion and inclusion criteria,and collected clinical and general information of all patients.Based on their willingness to participate and the order of visits,they were randomly divided into four groups,A,B,C,and D,with 25 in each group.Group A was given conventional anti-heart failure medications,and group B was received exercise rehabilitation treatmenton on the basis of group A..Group C received distant ischemic adaptation on the basis of group A.Group D received exercise rehabilitation and distant ischemic adaptation on the basis of group A.The respective groups were tested at the time of enrollment and 6 months later,which include Cardiac function classification,6 Minute Walk Test,plasma NT-proBNP and Cardiac color doppler echocardiography,the changes of each group were compared before and after treatment,and the corresponding data were analyzed and analyzed.At the same time,adverse events(such as aggravated heart failure,malignant arrhythmias,severe angina,sudden deathheart)occurred during distant ischemic adaptation and exercise rehabilitation treatment were recorded.Results:Before the intervention,there was no significant difference in the 6-mi nute parallel walking distance,NT-proBNP,and LVEF between the four groups o f patients A,B,C,and D(P>0.05).After the intervention,the groups B,C,an d D were not significantly different.The 6-minute parallel walking distance and LVEF were higher than those in group A,but LVESV,LVEDV,and NT-proBNP were lower than those in group A,and the differences were statistically signific ant(P<0.05);in group A,6-minute walking experiments,NT-ProBNP,LVEF,L VESV,and LVEDV were not statistically significant(P>0.05).In Groups B,C,and D,the parallel walking distance and LVEF increased 6 minutes after interve ntion,and NT-proBNP,LVESDV and LVEDV increased.It decreased after non-in tervention,and the difference was statistically significant(P<0.05).In group D,the parallel walking distance and LVEF at 6 minutes after intervention were high er than those in group B and C,but LVESV,LVEDV,and NT-proBNP were hig her than those in group B Groups C and C decreased,the difference was statistic ally significant(P<0.05).Conclusion:By performing remote ischemic conditioning in patients with chro nic heart failure combined with exercises rehabilitation in accordance with the pa tient’s condition,it can not only reduce the NT-proBNP level of patients,but als o improve cardiac function and exercise tolerance and affect long-term prognosis.It will become the second stage of chronic heart failure an integral part of prev ention.
