Research Purpose1.To understand the situation of extrauterine growth retardation in very low /extremely low birth weight infants, analyze its influencing factors, and provide a basis for reducing extrauterine growth retardation2.Study the effect of Bifidobacterium and lactobacillus on complications during hospitalization of very low / extremely low birth weight infants.Research objects and methods Sixty-six very low / extremely low birth weight infants who were admitted to The Second Affiliated Hospital of Jinan University and Huazhong University of Science and Technology Union Shenzhen Hospital from September 2015 to January 2017 were selected as research objects. And Retrospective analysis perinatal history, hospitalization, and complications of preterm infants included in the study.SPSS 20.0software was used for statistical analysis, and the t-test is used for measurement data for normal distribution,non-parametric test of rank transformation for rank conversion is used for measurement data for non-normal distribution, and the chi-square test is used for group comparisons of the enumeration data.Multifactorial logistic regression analysis was used to analyze the single factors(P<0.05)associated with extrauterine growth retardation and to determine the independent influencing factors of extrauterine growth retardation of infants with very low/extremely low birth weight; The preterm infants of this study, early admission, according to the principle of randomized double-blind divided into probiotic group B and group C probiotics,Probiotic group B was Bifidobacterium and another was lactobacillus, according to its growth data and complications, analysis the effects of probiotics in complications with extremely /very low birth weight infants.Result66 preterm infants were included in the study 1.Based on weight,35 cases of EUGR occurred at the time of discharge, with an incidence rate of 53%.Intrauterine growth retardation(IUGR)and gestational age at birth were risk factors for EUGR.The confidence interval is(2.02~10.31);Its odds ratio(OR)is 4.56,The risk of EUGR increased with gestational age at the same birth weight.time to regain weight are risk factors, its odds ratio(OR)is 1.56 and the 95%confidence interval is(1.19~2.05).Forty-five of 66 children were included with head circumference as the standard.EUGR was 6 with an incidence rate of 13.3%.The first day of intravenous nutrition may is a risk factor for EUGR.2.Among the 66 preterm infants included in the study, there were 48 cases of breast-feeding prematures, with a breast-feeding rate of 72.7%,and 43 cases of full breastfeeding prematures, with an full breast-feeding rate of 65.2%.In the full breastfeeding group,38 cases were treated with Human Milk Fortifier(HMF),human milk fortification rate 88.4%.There were no statistically significant differences in complications, the time to reach full enteral feeding, duration of intravenous nutrition, and duration of antibiotic treatment between the full breastfeeding group and the mixed feeding / premature infant formula feeding group. The incidence of EUGR between the full breastfeeding group and the mixed feeding / premature infant formula feeding group(P<0.050)was statistically different, and the average daily weight gains(P<0.001)was significantly different.3.By comparing the probiotics group with the placebo group, and the comparison of the probiotics group and the placebo group in different feeding methods,it was found that it had no statistically significant differences the in complications prevention duration of antibiotic treatment, the time to reach full enteral feeding, the duration of intravenous nutrition and the length of hospital stay.Conclusion1.Based on the weight,IUGR and gestational age and time to regain weight are risk factors for the occurrence of EUGR.The risk of EUGR increased with gestational age at the same birth weight.Based on the head circumference,Delayed first time of intravenous nutrition is a risk factor for EUGR.2.Very low / extremely low birth weight infants with full breastfeeding combined with HMF can obtain better daily weight gain and may reduce EUGR.3.This study did not find the effectiveness of Bifidobacterium and lactobacillus in the prevention of complications in very low / extremely low birth weight infants during hospitalization.
