Objective Objective to investigate the bleeding situation of interventional therapy combined with curettage and hysteroscopy in the treatment of scar pregnancy,and to evaluate the efficacy of interventional therapy in the treatment of first and second scar pregnancy.Methods 88 cases of cicatricial pregnancy confirmed by clinic were collected from Ningbo women’s and children’s hospital.To observe the related indexes:(1)the comparison of bleeding between interventional therapy and hysteroscopy.(2)(1)the comparison between the second intervention and the first intervention in the treatment of cicatricial pregnancy: the blood supply of the uterus,the embolization of the blood vessels,the rupture of the blood vessels and the time of the intervention;and(2)the comparison between the second intervention and the first intervention in the treatment of cicatricial pregnancy: the amount of bleeding and the success rate.Results(1)The bleeding volume of 80.3% patients with scar pregnancy treated by interventional therapy combined with hysteroscopy was less than 20 ml,78.2% patients with scar pregnancy treated by hysteroscopy was more than 20 ml(P < 0.01).(2)The blood supply rate of bilateral uterine artery decreased(48.5% vs 100%,P < 0.01)and the embolization rate of bilateral uterine artery decreased(42.4% vs 97%,P < 0.01)in the interventional treatment of second scar pregnancy compared with that of first scar pregnancy.The blood vessel was easily ruptured during superselective catheterization and embolization(15% vs 0%,P < 0.05),and the interventional operation time was prolonged(61.94 ± 15.85 min vs 40.45±9.47 min,P<0.01).(3)The amount of bleeding increased in the second intervention of scar pregnancy(86.82 ± 149.26 ml vs 6.06 ± 2.08 ml,P < 0.01),and the success rate of clearing uterus decreased(87.9% vs 100%,P < 0.05).Conclusion Preoperative interventional embolization can effectively reduce the bleeding in the treatment of cicatricial pregnancy,but it is more difficult to perform the second interventional therapy for the second cicatricial pregnancy,and the risk of bleeding is still high in the process of postoperative uterine cleaning,so it is necessary to make full preparation before the operation.
