Objective:To study related factors of perioperative hidded blood loss in elderly intertrochanteric fractures,to guide the treatment process and speed up postoperative recovery,to reduce the adverse effects of hidden blood loss,to improve prognosis and reduce mortality,and finally to provide theoretical guidance for perioperative clinical treatment of intertrochanteric fractures in elderly patients.Method:The clinical data of 80 patients with intertrochanteric fractures who were hospitalized for surgical treatment in Ning Bo No.2 hospital from September 2018 to August 2019were included in the retrospective study.Among them,there were 32 males and 48 females,Aged62~99 years,with an average age of(78.6±8.9)years old;Sarcopenia in 36 cases;osteoporosis in49 cases.The Gross equation was used to calculate the total blood volume,total blood loss,obvious blood loss and hidden blood loss in the perioperative period.The effects of age,gender,BMI,sarcopenia,osteoporosis,operation methods,operation time and anesthesia methods on the hidden blood loss during perioperative period were analyzed.SPSS18.0 statistical software was used to process them.Data were recorded in the form of X±s.Graph Pad Prism 8 was used to visualize the data.The related factors of occult blood loss after intertrochanteric fracture in the elderly patients were analyzed by multiple linear regression analysis,and the results were tested by T or variance test.Test level:P<0.05 indicates that the comparison results are statistically significant.Results:Age,Osteoporosis,Sarcopenia and Operation methods had statistical significance in total blood loss and hidden blood loss after operation(P<0.05);while the gender differences,BMI,operation time and anesthesia methods had no statistically significant in hidden blood loss on elderly patients with intertrochanteric fracture(P>0.05).The hidden blood loss was set as the dependent variable(Y)and each influencing factor as the independent variable(X),The results showed that age(X1),osteoporosis(X2),sarcopenia(X3),and surgical method(X4)were positively correlated with the hidden blood loss in the perioperative period(P<0.05).The regression model was established according to the results of multiple linear regression analysis:381.029+99.102X1+61.548X2+56.671X3+46.056X4=YConclusion:The occurrence of hidden blood loss after intertrochanteric fractures in elderly patients is mainly related to age,osteoporosis,sarcopenia and surgical methods.It can be used to estimate and intervene the blood loss in elderly patients with intertrochanteric fracture,so as to timely replenish blood volume,reduce the pain and complications,reduce the average hospitalization time and cost,accelerate the recovery and regain previous mobility as soon as possible.
