Objective: Functional Magnetic Resonance Imaging was used to extract the characteristics of brain function in patients with Knee Osteoarthritis compared with healthy subjects,to observe the effect of ozone water on brain function in patients with KOA,and to explore the central regulation mechanism of ozone water in the treatment of KOA,so as to find a theoretical basis for the clinical effect of ozone water in the treatment of KOA.Methods: KOA patients and healthy subjects who met the inclusion criteria were divided into experimental group and control group.Patients with KOA were treated with ozone water joint cavity irrigation and acupoint injection.acupuncture points xue hai(SP10),liang qiu(ST34),yin ling quan(SP9),yang ling quan(GB34),san yin jiao(SP6)were selected on the affected side.and 2ml/ were selected for injection,once a week for 3 times.Clinical evaluation and resting state f MRI scanning were performed before and after treatment,while healthy subjects were scanned only once for f MRI as control.By using the method of regional homogeneity analysis,the brain regions with different Re Ho between the two groups and the brain regions with different Re Ho in KOA patients before and after ozone water treatment were obtained,and the changes of Re Ho and clinical scale scores before and after ozone water treatment were analyzed by Pearson.Results: 1.There is no significant difference in baseline between KOA patients and healthy subjects,so it is comparable(P < 0.05).2.Compared with those before ozone water treatment,the VAS score of KOA patients decreased significantly and Lysholm score increased significantly after treatment(P < 0.05,It is statistically significant).3.Compared with the control group,the Re Ho values of right superior cerebellum,right inferior cerebellum,right superior occipital gyrus,right cuneate lobe,right angular gyrus and right middle occipital gyrus increased in KOA patients(P<0.05);while Re Ho values of left inferior temporal gyrus,left middle temporal gyrus,left superior temporal gyrus,left central sulcus cover,right middle frontal gyrus and right precentral gyrus decreased(P <0.05).4.After treatment with ozone water,the Reho values of right superior occipital gyrus,right middle occipital gyrus,right cuneate lobe and right angular gyrus increased in KOA patients(P <0.05);the Reho values of right central sulcus cover,right insular lobe,right superior temporal gyrus,right transverse temporal gyrus,right precentral gyrus,left supplementary motor area and right supplementary motor area decreased(P<0.05);in addition,the change of Re Ho in the right insular lobe was negatively correlated with the change of VAS(r=-0.657,P=0.003).Conclusion: 1.Acupoint injection of ozone water combined with articular cavity irrigation can improve the pain and functional activity of patients with KOA,and the clinical effect is significant.2.There was a significant difference in local consistency between KOA patients and healthy subjects at rest.3.The clinical effect of ozone water is reflected by regulating the function of temporal lobe,parietal lobe,frontal lobe,occipital lobe and other brain regions,while insular is the target brain area of ozone water therapy for KOA.
