Objective Comparison of the clinical curative effect of contrast-enhanced ultrasound and conventional ultrasound guided PCNL for the treatment of nondilated collecting system kidney stones.Methods(1)Form October 2018 to December 2019,Patients treated with PCNL for kidney stones in the second people’s hospital of the three gorges university were selected to be included in this prospective study.SPSS 20.0 software was used to generate random Numbers and randomly divide the patients into two groups.The experimental group underwent PCNL under the guidance of contrast-enhanced ultrasound,while the control group underwent PCNL under the guidance of conventional ultrasound.(2)The basic situation(including gender,age,BMI,stone size,stone location and stone distribution)of the two groups were compared to determine the comparability.(3)Intraoperative and postoperative indexes(including the number of attempts for successful puncture,the time to successful puncture,operation time,number of channels,stone clearance rate,complications,hospital length of stay and postoperative hemoglobin concentration)of the two groups were compared to determine whether there were statistical differences,and the clinical effects of two different puncture technique were compared.Results(1)A total of 60 patients were included in this study,30 cases in each of the contrast-enhanced ultrasound group and the conventional ultrasound group.There was no significant statistical difference in the basic situation(including age,sex,BMI,stone size,stone location and stone distribution)of the two groups(P﹥0.05).(2)The results of the two groups were compared.PCNL was performed successfully in all 60 patients.Contrast-enhanced ultrasound group was superior to the conventional ultrasound in the success rate of one puncture(80%vs 53.3%),the time to successful puncture(8.94±2.01 vs10.64±3.54)min,hospital length of stay(9.13±1.48 vs 11.33±3.25)d,complications and postoperative hemoglobin concentration(126.2±13.6 vs 117.3±19.4)g/L,and the difference had statistical significance(P<0.05).In terms of operation time(84.83±19.07 vs91.97±17.57)min,number of double channels(1 vs 4),stone clearance rate(86.7%vs73.3),postoperative WBC(8.17±2.17 vs 8.33±2.59)×10~9/L,and postoperative Scr(85.5±23.9 vs 88.8±23.4)μmol/L,there was no statistical significant difference between the two groups(P﹥0.05).Conclusion Contrast-enhanced ultrasound technique can improve the visibility of nondilated collecting system,facilitate the surgeon to select suitable target calyces and identify the renal calices fornix.Compared with the Conventional ultrasound guidance technique,Contrast-enhanced ultrasound technique can increase the success rate of one puncture,reduce intraoperative and postoperative bleeding,reduce postoperative complications,and reduce the hospital length of stay,which may provide a new choice for PCNL.
