Objective: To investigate the clinical significance of serum hapatocyte growth factor(HGF)in children with Mycoplasma pneumoniae pneumonia(MPP).Methods: 1.Study object: From January 2014 to June 2015,children with pneumonia in Nankai Hospital of Tianjin were selected as the study object.They were aged from 3 to 14 years old.There were 107 cases in the group,all of them had no other systemic diseases.The age,sex,height,respiration,pulse,blood oxygen saturation and other general data of all children with pneumonia after admission were collected.Blood and sputum culture,serum MP Ig M(Mycoplasma pneumatia immunoglobulin M,MP Ig M)and lung imaging were detected respectively.After the test results were reported,they were divided into MPP group and non MPP group,including 65 cases in MPP group and 42 cases in non MPP group.If the serum MP Ig M is greater than or equal to 1:160,or if the titer of MP Ab is more than 4 times(increased or decreased)in the acute stage and recovery stage of children,it is the critical value of MP positive.In MPP group,according to the condition evaluation criteria,it can be divided into non severe MPP group(38 cases)and severe MPP group(27 cases).Another 30 healthy control group were set up,which were healthy children matched with age and gender.Serum HGF was measured in all the children with pneumonia and 48 hours in MPP group to evaluate the therapeutic effect.2.Research methods: Detection of HGF in serum by ELISA;Detection of HGF in serum by ELISA 3.Evaluation indexes: Serum MP-Ig M,Serum HGF,C-reactive protein(CRP),lactate dehydrogenase(LDH),etc 4.Statistical methods SPSS17.0 system was used for statistical analysis.The normal distribution data were expressed by ± SD,and the non normal distribution data were expressed by median(quartile range).The comparison between groups was performed by Mann Whitney U test and Wilcoxon test before and after treatment in the same group.ROC curve and area under the curve(AUC)were used to evaluate the therapeutic value of HGF and CRP in the prediction of macrolides in the treatment of MPP.P < 0.05 means the difference is statistically significant.Results: 1.The HGF of healthy control group,MPP group and non-mpp group showed a gradually increasing trend(4174.3 ± 247.6)pg/m L,(6507.1 ± 1895.9)pg/m L and(7128.7 ± 1990.2)pg/m L,and the comparison between the three groups was statistically significant(P<0.05).2.Serum HGF(7668.8± 1961.5)pg/m L in severe MPP group was significantly higher than that in non-severe MPP group(5681.7±1 352.7)pg/m L,and the comparison between the two groups was statistically significant(P<0.05).3.Serum CRP[28.5(5.5,30.1)]mg/L in children with severe MPP group was significantly higher than that in children with non-severe MPP group [19.5(2.4,25.0)]mg/L,and the comparison between the two groups was statistically significant(P<0.05).4.Serum LDH(337.0 ±120.3)IU/L in severe MPP group was significantly higher than that in non-severe MPP group(256.9 ±54.4)IU/L,and the comparison between the two groups was statistically significant(P<0.05).5.Among all children with MPP,azithromycin was effective in 42 cases(64.6%)and ineffective in 23 cases(35.4%).HGF was(4964.3± 883.0)pg/m L and CRP was 5.6(1.2-13.9)mg/L in the effective group 48 hours after admission.After 48 hours of treatment,HGF was(7470.9±2463.9)pg/m L and CRP was 14.3(3.9-39.9)mg/L.The difference before and after treatment was statistically significant(p < 0.05).6.In the ROC curve of the predictive value of HGF and CRP on the treatment of children with MPP,the area under the HGF curve(AUC)was 0.914,and the area under the CRP curve(AUC)was 0.833.The difference was statistically significant(p < 0.05).Conclusion:1.HGF it participated in the occurrence of MPP and helped to evaluate the severity of the disease.2.HGF It has certain guiding significance for the evaluation of clinical treatment effect of MPP children.
