Objective1.The introduction of foreign Cough Assessment Test(COAT)as a research tool to understand the current status of the severity of chronic cough patients in China and the status of psychological distress under different levels of cough.2.Through this study,under the guidance of a multi-factor system theory model of psychological stress,the causes of psychological distress in patients with chronic cough were analyzed,and the influencing factors of psychological distress in patients were analyzed to provide a theoretical basis for clinically reducing psychological distress in patients with chronic cough.Methods1.Study one: Chinese version of cough assessment test(COAT)reliability and validity test.108 cases of chronic cough patients in a second-class first-class hospital in Tianjin were selected as the subjects of investigation.After obtaining the authorization of the original author of COAT,Professor Jang,the introduction procedure was strictly followed in accordance with the standard international scale,that is,"translation-back translation-cultural debugging-pre The method of“experimentalization” requires the Chinese COAT scale to be independently reviewed and revised by two clinical bilingual nursing experts,and then the Chinese version of the COAT scale is finally formed after discussion;30 patients were selected for pre-experiment,and the patient ’s general data survey was completed.The shortcomings and problems in the investigation of the COAT scale of the version ensure the feasibility of the subject.The patients with chronic cough were investigated by convenience sampling and Rasch analysis was used for reliability and validity test.The data were analyzed using the rating scale model,including one-dimensional test,item fitting test,measurement reliability(separation index and reliability of subjects,separation index and reliability of items),validity,correlation analysis and white diagram.2.Study two: analysis of psychological distress and influencing factors in patients with chronic cough.Choice in May 2019-November 2019 tianjin a level 2grade a hospital,230 cases of patients with chronic cough as the object,designed the general information questionnaire investigation,including demographic data,disease related data and cough assessment,medical coping questionnaire,psychological distress scale,cough specific quality of life scale.Single factor analysis and correlation analysis were used to explore the psychological distress of patients with chronic cough with different characteristics,and the influencing factors of psychological distress of patients with chronic cough were explored by means of multiple stepstep regression analysis.Results1.The introduction of the scale,the Chinese version of the cough assessment test was determined to include 5 items through the chinese-version of the scale COAT.The overall item separation coefficient of the scale was 10.03 and the reliability was0.99.The separation coefficient was 2.03 and the reliability was 0.81.There was a significant difference in the average score of the two groups(F=361.329,P<0.05).The value of the correlation coefficient between the scores of the two groups was between 0.361 and 0.589,and the value of the correlation coefficient between the scores of each group and the total score of the test was between 0.643 and 0.802.In the white diagram,the ability of the subjects on the left is evenly distributed and the items on the right are evenly distributed.2.In this study,230 patients with chronic cough were included,including 136males(59.13%)and 94 females(40.87%),with an average age of(56.74±4.69).The duration of cough ranged from 1 to 5 years,accounting for 49.13%.The severity of cough affects the level of psychological distress.3.Correlation analysis: psychological distress level of patients with chronic cough was negatively correlated with avoidance and surrender in coping styles(P<0.05).It was positively correlated with cough score,quality of life score and coping style(P<0.05).The results of univariate and multivariate analysis showed that the influencing factors of psychological distress in patients with chronic cough included complications,duration of cough,total score of quality of life,cough score and coping style.Conclusions1.Through the method of "translation-back translation-cultural debugging-pre-experiment",the Chinese version of the COAT scale was finally formed after the review and modification by experts,and the Chinese version of the COAT scale had a good reliability and validity,which laid a foundation for the follow-up research.2.In this study,most patients with chronic cough suffered from above average psychological distress.3.The psychological distress of patients with chronic cough was more obvious,and complications,duration of cough,total score of quality of life,cough score and coping style were all influencing factors.Therefore,effective and targeted measures can be formulated for intervention according to its influencing factors,so as to alleviate the negative emotions of patients with chronic cough,reduce their psychological distress and improve their quality of life.
