Objective: This paper compared the differences in risk factors between CSVD patients and control group with cranial magnetic resonance imaging(MRI)and multiple cognitive scale evaluations in the patient group and control group,and compared the characteristics of cognitive impairment in patients with high levels of cerebral white matter,and compared the differences in cognitive impairment in patients with different severity of cerebral white matter high-signal.In order to explore the mechanism of disease physiological change in Patients of CSVD,it is beneficial to carry out early intervention on cerebral vascular disease in a targeted manner,and find new methods and means for early detection of high brain white matter signal,with a view to giving early intervention to cognitive impairment in patients with high brain white matter signal,and improving the prognosis of patients.Methods: A total of 100 patients were randomly collected in our hospital,including 55 CSVD patients(including 37 patients with high white matter signal)and 45 controls.Skull magnetic resonance angiography(MRA)was completed from November 2018 to December 2019 to exclude large intracranial and extracranial stenosis and to perform cranial MRI examination,including T1 imaging(T1WI),T2 imaging(T2WI),diffusion imaging(DWI),T2 fluid attenuation reversal recovery sequence(FLAIR),magnetic sensitivity imaging(SWI)and diffusion(diffusion)tensor imaging(diffusion).TI).the small cerebral vascular disease group was divided into asymptomatic lumen infarction(SLI),cerebral microbleeds(CMB),and high white matter signaling(WMHs).The general data of cerebral small vascular disease group and control group,including sex,age,whether combined with hypertension,diabetes,hyperlipidemia and other basic diseases,as well as lifestyle such as smoking,drinking,physical activity and so on,were analyzed.The white matter high signal group and the control group were tested AD8、Mini-cog、Mo CA scale,DS(digital breadth test),VFT(language fluency test),CTT.-A and CTT-B(wired test A and B),VLT(auditory vocabulary learning test),SCWT(Stroop Color-Word Test,Stroop Color-Word Test),Boston nomenclature test),sleep quality,PHQ-9(simple self-test depression scale),generalized anxiety scale)to analyze the characteristics of cognitive impairment in the white matter high signal group.Comparing cognitive scale scores,FA values of DTI imaging and FA values of DTI imaging in different areas of severe brain white matter high signal,the characteristics of cognitive impairment of high signalof in brain white matter and the severity of different injury sites were analyzed.Result:1.The demographic and clinical data of the study subjects: there was no statistical difference in sex and alcohol consumption between the control group and the cerebral small vascular disease group(P>0.05).were differences in age,hypertension,diabetes,coronary heart disease,smoking and physical activity between the two groups(P>0.05).2.The total score,DS-positive back,DS-reverse back,and CTT-A、CTT-B scores of the brain white matter high signal group compared with the control group were significantly different(P<0.05).There was no statistical difference between the high signal group of brain white matter and the control group in AD8,Mini-cog,VFT,AVLT,and SCWT.(P>0.05).3.There was significant difference PHQ-9 the self-measured depression scale between the white matter hypersignal group and the control group(P<0.05),while there was no statistical difference in anxiety self-rating scale GAD-7 and sleep quality(P<0.05).4.The white matter high signal group was divided into light,medium and heavy groups according to the Fazekas grade,with a total score of 1-2 points,3-4 points and 5-6 points.The Mo CA scores of the three groups were compared.The results showed that there was difference between the two groups(P<0.05),and there was no significant difference between the other groups(P>0.05).5.White matter high signal group are all head core magnetic DTI imaging examination,compared to the light medium white matter high signal group and heavy group white matter high signal,between the two groups FA value in the front foot of the side ventricle is statistically significant(P <0.05),and the severe brain white mass high signal group lateral ventricle pre-angle and the difference in FA and ADC values between the back corner of the lateral ventricle and the center of the semi-egg circle and the rear corner of the lateral ventricle was statistically significant(P< 0.05),the age difference between the two groups was statistically significant(P <0.05),and there were significant differences between the two groups in DS-positive back and CTT-B(P <0.05).Conclusion:1.Compared with the control group,hypertension,diabetes and coronary heart disease were all risk factors for cerebral vascular disease,and smoking was more likely to occur.The incidence of cerebrovascular disease decreased significantly in people who regularly engaged in physical activity2.Depression was more severe in the white matter hypersignal group.The cognitive impairment in patients with high white matter signal was mainly manifested in decreased executive ability and poor attention,while the cognitive impairment in memory and language function was not obvious.3.The Mo CA score was negatively correlated with the severity of high white matter signal.According to the Fazekas grade,the lower the Mo CA score and the lowest in the severe white matter high signal group.4.The FA value of MR DTI imaging was lower in patients with lighter moderate whiteness signal sonin softened in patients with severe white matter signal,indicating that the damage was more serious in this area,and that the damage to the front algeum of the side ventricle in patients with severe whiteness signal was more serious than the damage to the front of the semi-egg center and the lateral ventricle.Patients with severe white high signal smaller moderate white matter high signal patients are older and have poorer attention and execution function.
