Research Purpose:To describe the status quo of self-perceived burden,quality of life,medical coping style and social support of patients with chronic lower limb wounds;To analyze the relationship between self-perceived burden and life quality,medical coping style and social support in patients with lower limb chronic wound.To explore the influencing factors of self-perceived burden of patients with chronic lower extremity wounds,and to provide reference strategies for effectively reducing self-perceived burden of patients in clinical practice.Research Methods:Part Ⅰ: Questionnaire survey on the status quo of self-perceived burden of patients with lower extremity chronic wound and its influencing factors.(1)Using the convenience sampling method,the selection in November 2019 to November2020,zhejiang province,a city 3 armour hospital vascular surgery and wound colostomy outpatient clinic of 198 cases of patients with chronic wounds in the investigation of lower limb,research tools,including: general information questionnaire and self feeling the burden scale and Cardiff wound impact scale,medical coping mode questionnaire,social support scale.(2)Use Excel to collect data,establish a database,using SPSS22.0 software to analyze data,descriptive statistical analysis to basic description of general information,using the single factor variance analysis,independent sample t-test,Pearson correlation analysis and multiple linear regression analysis methods to analyze the factors influencing patients feel burden yourself.P<0.05 was statistically significant.Part Ⅱ: A qualitative study on the influencing factors of self-perceived burden in patients with lower extremity chronic wound.A total of 12 patients in the vascular surgery department and wound stomy outpatient department of a Grade A hospital in a city of Zhejiang Province from November 2020 to January2021 were selected as the research objects by purpose sampling method.According to the literature review and pre-interview design interview outline,semi-structured in-depth interviews were conducted on the patients.Colaizzi seven-step analysis method was used to analyze the data.The influencing factors of self-perceived burden of patients with chronic lower limb wounds were further clarified.Research Results:1.The total score of self-perceived burden of patients with lower extremity chronic wound was(29.94±7.93)points,in which the score of physical burden was(15.79 ± 3.43)points,the score of emotional burden was(11.53±3.49)points,and the score of economic burden was(2.93±1.36)points.2.Quality of life score of patients with lower limb chronic wound was(47.53±9.93);The scores of each dimension of medical coping style were:(17.79±2.77)points in face,(19.69±1.98)points in avoidance,and(9.73 ± 3.59)points in surrender.The total social support score was(32.56±5.23)points,including(15.07±2.94)points for subjective support,(10.05±1.82)points for objective support and(7.44±1.79)points for utilization of support.3.Univariate analysis showed that the self-perceived burden of patients with lower extremity chronic wound had differences in culture,occupation,residence,self-care ability,monthly income,medical payment method,wound recurrence,wound stage,wound duration and history of complicated diseases(p<0.05).4.Correlation analysis showed that quality of life(r=-0.905,P<0.01),medical coping style(r=-0.753,P<0.01),social support(r=-0.577,P<0.01)were negatively correlated with self-perceived burden of patients with lower limb chronic wound;Medical coping style: avoidance(r=0.520,P< 0.01),and medical coping style: yield(r=0.886,P<0.01)were positively correlated with self-perceived burden.5.Multiple linear regression analysis showed that occupation,wound area,quality of life(physical symptoms and daily life),avoidance,submission,subjective support,and support utilization entered the regression equation.6.Four themes were extracted from the data collected after the semi-structured interview:insufficient awareness of disease,heavy economic burden,guilt,and multiple emotional experiences.Research Conclusion:1.The incidence of self-perceived burden in patients with lower extremity chronic wounds is relatively high,and the overall level is mild or moderate.2.Occupation,wound area,quality of life(physical symptoms and daily life),avoidance of medical coping styles,yield dimension,subjective support of social support,and support utilization dimension were the main influencing factors of self-perceived burden of patients with lower extremity chronic wound.3.The results of qualitative studies suggest that clinical medical staff should formulate specific intervention measures in terms of disease cognition,economic burden,guilt and multiple emotional experience,so as to improve the status quo of self-perceived burden of patients with lower limb chronic wounds.
