| Research Purpose:Guiding by the JBI evidence-based health care model updated in 2016,this study intends to construct a fall prevention program for hospitalized elderly patients with mental disorders that is in line with native conditions and close to the clinic on the base of integrating the best literature evidence,clinical situational interview data and results of multiple rounds of expert consultation to provide geriatric psychiatrists with an optimal fall prevention strategy.Research Methods:Under the guidance of JBI evidence-based theory in 2016,literature evidence on fall prevention in hospitalized elderly patients with mental disorders were extracted through comprehensive retrieval and evaluation of subject-related Chinese and English literatures.Focus interviews with front-line clinical nurses and support crew were arranged to obtain the current effective means for fall prevention in clinical situation and to improve the fall program items.By selecting nursing experts with rich clinical experience to conducted two rounds of consulting to improve the fall prevention program for hospitalized elderly patients with mental disorders.Research Results:Evidence of 63 English literature and 29 Chinese literature were summarized,interviews were carried out with 32 of medical care and support staff,two rounds of consulting were conducted by 20 geriatric psychiatric nursing specialists.Finally,the fall prevention program was constructed,including 4 first level items: fall risk evaluation,fall risk administration,environment management and cultural and institutional construction;13 secondary items: admission evaluation,dynamic evaluation of hospitalization,personalized strategies,multi-disciplinary and multi-department cooperation,drug management,ward environment evaluation and improvement,appropriate ratio of medical care,reasonable protective restraint,using of auxiliary equipment,quality improvement and so on;57 three-level items.Research Conclusion:The fall prevention program constructed in this study is a nursing practice combined best literature evidence,clinical scenario,expert arguments and judgments,it is comprehensive and specific,which can not only provide guidance for fall evaluation in geriatric psychiatry department,reference for managers to develop localized fall management programs,but also provide reference for the establishment of a more effective fall prevention and control system in clinical practice.It has certain scientificity,reliability and feasibility,and can be promoted in clinical trial. |
