| Research Purpose:Through the investigation of nurses’ palliative care knowledge,attitudes and behavior elements under the hospice shared care model,we can understand the status quo of nurses’ palliative care knowledge,attitudes and behaviors,and further analyze the factors that affect nurses’ nursing knowledge,attitudes and behaviors.To find out the weak links and existing problems in nurses’ knowledge,attitude and behavior of palliative care.Based on the research results,relevant suggestions are put forward to provide a theoretical basis for the formulation of relevant measures for hospital nursing managers and the improvement of the quality of hospice care for nurses,so as to lay a foundation for providing high-quality nursing services for patients at the end of life.Research Methods:The first part is to compile the questionnaire on knowledge,attitude and behavior of nurses in palliative care,conduct two rounds of expert consultation,complete the revision of the questionnaire items,and complete the test of the reliability and validity of the questionnaire through pre-survey,and finally form a formal questionnaire.In the second part,a questionnaire survey was conducted,with nurses who participated in the hospice shared care model in a 3-A grade hospitals in Hangzhou as the research objects.Questionnaires were sent to 330 subjects in the form of on-site questionnaires.After the questionnaire was collected,the questionnaire was checked twice,and the questionnaire data after the two-person screening was entered into the Excel table to ensure the accuracy of the data entry.Use SPSS23.0 software for statistical analysis of the entered data.The general demographic data of nurses are expressed in the form of frequency and percentage;the scores of nurses’ palliative care knowledge,attitudes,and behaviors are expressed as percentages,mean±standard deviation;the influence of different demographic characteristics on nurses’ palliative care knowledge,attitudes,and behavior scores is expressed by t-test,single factor analysis of variance,multiple linear stepwise regression methods are used for multi-factor analysis,and nurses’ palliative care knowledge,attitudes,and behavioral dimensions are related Pearson correlation analysis is used to express the sex,and the influencing factors are judged based on the results.Research Results:1.The preparation and evaluation of the questionnaire: After two consultations with experts,a formal questionnaire was formed.The questionnaire has 45 items and 3 dimensions,namely knowledge dimension,attitude dimension,and behavior dimension.The Cronbach’s coefficients of the three dimensions in this questionnaire were 0.714,0.758,0.820,respectively,Cronbach’s coefficients in the total questionnaire was 0.851;the retest reliability were 0.801,0.852,0.887,0.854 and the content validity(S-CVI/Ave)of the questionnaire is 0.909.The reliability and validity of the questionnaire is good.2.In this study,a total of 330 questionnaires were distributed and 330 were returned.The recovery rate was 100%.All questionnaires are valid after being checked by two persons,and the effective rate is 100%.3.The full score of the hospice knowledge dimension of nurses was 20 points,and the average score was(15.63±1.81)points.The knowledge mastery of nurses was at a medium level,For item 8 of knowledge dimension,“the palliative care ward of end-of-life patients patients is no need for discharge plan”,and the correct rate is the lowest,only 38.48%,Item 14: “Give painkillers only when patients start to feel pain.”,the accuracy rate of avoiding addiction or increasing dose is the second lowest,only 55.45%;the attitude dimension of hospice care for nurses was at a relatively high level.The full score of the attitude dimension in this questionnaire was 85 points,and the average score was(69.79±7.96).For item 10 of attitude dimension,“I think I have enough knowledge to discuss the topic of death with patients and their families”,and got the lowest score,with an average score of(3.19±0.95)points;the practice dimension of hospice care for nurses was at a moderate level.The full score of the practice dimension in this questionnaire was 40 points,and the average score was(30.46±3.74)points.Among them,the practice dimension item 5 “Helping and guiding dying patients to establish or meet reasonable social needs”has the lowest score,which is(2.62±0.90)points.4.After the normality test,the knowledge,attitude,and behavior questionnaire scores of nurses’ palliative care showed a normal distribution.For the comparison of the two groups of data,the t test was used.For the data of the two groups and above,if the data has homogeneous variance,A one-way analysis of variance(ANOVA)was used for comparison.The survey results of this research show: the job title,education level,working years in general demographic data,whether you have participated in on-the-job training of tranquility care,the duration of participating in tranquility care training courses(training classes or conferences)within two years,and whether you are willing to participate in tranquility care Nursing volunteer services and other factors have statistically significant effects on nurses’ knowledge mastery of palliative care(P<0.05);department of general demographic information,marital status,whether working years,participated in the hospice care within two years of on-the-job training,to participate in hospice care training courses(classes or conference),willing to transfer to the hospice care ward,is willing to participate in volunteer service of hospice care attitude toward hospice care nurse levels affect the statistically significant(P<0.05);department of general demographic data,different ages,titles,whether in hospice care within two years of on-the-job training,to participate in hospice care training courses(classes or conference),to take care of the home seriously ill patients experience,and is willing to participate in volunteer service of hospice care to nurse the behavior of the hospice care practice effect statistically significant(P<0.05).5.The results of multiple linear stepwise regression analysis showed that the education level and professional title of nurses in two years participated in hospice care training course length(training class or conference)was the main factor affecting the scores of hospice care knowledge of nurses.Whether the nurse was willing to transfer to the palliative care ward,whether the nurse was willing to participate in the palliative care volunteer service,the length of time(training class or conference)attended the palliative care training course in two years and the working years had statistical significance on the scores of the nurses’ attitude towards palliative care.The department and title of the nurse and the experience of caring for the sick patients at home had statistical significance on the scores of hospice care practice of nurses.6.Pearson correlation analysis shows: attitude dimension and practice dimension are positively correlated(r=0.406,P=0.000);knowledge dimension is not correlated with attitude dimension(r=0.091,P=0.097);knowledge dimension is not correlated with practice dimension(r=0.024,P=0.664).Research Conclusion:1.The survey results of this study showed that the knowledge and practice level of hospice care of nurses participating in the hospice shared cared model were above the medium level,and the attitude level was at the high level.2.In this survey,the main influencing factors of nurses’ knowledge of palliative care were:education level,professional title,working years,length of time(training class or conference)attended palliative care training courses within two years.The influencing factors of the nurse’s attitude towards palliative care include: the working years of the nurse,whether the nurse is willing to transfer to the palliative care ward,whether the nurse is willing to participate in the palliative care volunteer service,the length of the palliative care training course(training class or conference)within two years.The influencing factors of nurse’s palliative care practice include:nurse’s department,professional title and experience in caring for seriously ill patients at home.3.The correlation analysis results among the knowledge,attitude and practice of nurses in hospice care showed that there was a positive correlation between attitude and practice dimensions. |
