| The defects of speech production are not only the problems of articulation,but also the defects of speech rhythm,breathing,vocalization and resonance.Speech fluency refers to an individual’s fluency and proficiency in using spoken language to convey information.It is mainly manifested in speech acts such as speech rate,pauses,prolongation,repetition.It is an important standard to measure the individual’s speech rhythm function.Good speech fluency function can directly improve the speaker’s speech clarity and the listener’s speech intelligibility.The speech fluency disorder of individuals usually appears as the abnormal forms of slow speech rate,improper pause,prolongation,abnormal repetition,meaningless spoken language,which seriously affects the normal social communication activities.Because of the non-progressive brain injury involving speech muscle groups related to speech production process,most children with spastic cerebral palsy cannot complete accurate movement or movement process in the pronunciation process,which leads to speech fluency disorder.However,the speech therapy of children with cerebral palsy is in its infancy.The clinical research on speech fluency disorder in children with spastic cerebral palsy is only on the empirical level,and there is no empirical study.The purpose of this study is to explore the clinical characteristics of speech fluency disorder in children with spastic cerebral palsy and to propose the treatment of speech fluency disorder in children with spastic cerebral palsy——Game Real-time Accent Method(game RAM),To formulate the specific content and implementation process of the method and its supporting rehabilitation equipment,and to verify its effect through clinical empirical research.The study is divided into two parts.In the first part,through the comparative study of speech fluency between children with spastic cerebral palsy and ordinary children,the typical characteristics of speech fluency disorder in children with spastic cerebral palsy in the form of imitation of following language task were systematically discussed.In the second part,the problems of speech fluency disorder in children with spastic cerebral palsy were solved as the foothold,and some suggestions were put forward Objective to explore Game Real-time Accent Method and clinical application.The details are as follows:1.The characteristics of speech fluency disorder in children with spastic cerebral palsyIn Study 1,the clinical features of speech fluency disorder in children with spastic cerebral palsy were studied.Under the control of age,gender,language and other factors,44 children aged5-7 and 44 children with spastic cerebral palsy under the background of mandarin were selected.The adaptation of singing by Liang Junbo was used as the test material.The language samples of the subjects were collected in the form of imitation following language task,and their speech fluency characteristics were analyzed.The independent variables were spastic cerebral palsy children and normal children.The dependent variables were speech rate(speech rate,articulation rate),pause(total pause times,abnormal pause times,average pause duration,average abnormal pause duration,abnormal pause duration)The typical differences of speech rate,pause,prolongation and repetition were compared between the two groups.The results show that spastic cerebral palsy children’s speech rate and articulation rate are significantly lower than ordinary children’s,which indicates that spastic cerebral palsy children’s speaking rate is slow,and there is the problem of abnormal speaking rate;spastic cerebral palsy children’s average scores of pause times,pause duration,abnormal pause times and abnormal pause duration are significantly higher than ordinary children’s,which indicates that spastic cerebral palsy children’s speech rate is abnormal The results showed that children with spastic cerebral palsy had obvious abnormal pause in oral expression,with the problem of abnormal pause;children with spastic cerebral palsy had significantly higher average scores of prolongation times and prolongation duration than ordinary children,which indicated that children with spastic cerebral palsy had obvious prolongation in oral expression,with the problem of abnormal prolongation;children with spastic cerebral palsy had significantly higher average scores of repetition times than ordinary children The results show that the repetition of children with spastic cerebral palsy is obvious and there is abnormal repetition.To sum up,it shows that children with spastic cerebral palsy have obvious speech fluency disorder,which is manifested in slow speech rate,abnormal pause,abnormal prolongation and abnormal repetition.2.Study on the treatment of speech fluency disorder in children with spastic cerebral palsy by Game Real-time Accent MethodBased on the characteristics of speech fluency disorder in children with spastic cerebral palsy,according to the basic principles of Accent Method,study 2 proposed " Game Real-time Accent Method " to solve speech fluency disorder in children with spastic cerebral palsy,and formulated the specific content and implementation process of Game Real-time Accent Method.The prominent feature of this method is the real-time of audio-visual feedback,which enables individuals to detect their own speech acts simultaneously during training,and helps to adjust their own speech acts timely and accurately.In this study,44 children aged 5-7 with spastic cerebral palsy under the background of mandarin were selected and divided into experimental group and control group.The results showed that the speech rate and articulation rate of children with spastic cerebral palsy in the experimental group were significantly improved,the number of abnormal pauses and the duration of abnormal pauses were reduced,the number of prolongation and the duration of prolongation were reduced,and the number of repetitions was reduced;On the contrary,there was no significant change in the parameters of children with spastic cerebral palsy in the control group.The experimental results verify the effectiveness of the Game Real-time Accent Method for speech fluency in children with spastic cerebral palsy.The innovations of this study are as follows: 1.Through four evaluation indexes of speech rate,pause,prolongation and repetition,and nine quantitative evaluation parameters,the characteristics of speech fluency disorder of children with spastic cerebral palsy before and after5-7 years of age were comprehensively discussed;2.For the first time,the treatment method of Game Real-time Accent Method was proposed to solve the speech fluency disorder of children with spastic cerebral palsy Methods based on modern speech rehabilitation equipment,it can present real-time speech feedback cues of individual visual and auditory dual channel stimulation in real time,so that individuals can simultaneously detect abnormal speech behaviors of their own speech fluency disorder during training,which has important reference significance for the clinical treatment evaluation of speech fluency disorder in children with spastic cerebral palsy in the future. |
