Application Of Case Management In Rehabilitation Of Elderly Stroke Patients With Hemiplegia

With the continuous development of social work,on the basis of foreign countries,China has tried to explore the service mode adapted to the local culture and environment in different fields.In such a good external environment,the author also thought about how social work could provide services for the rehabilitation of stroke hemiplegia patients with complex needs through the internship in the hospital,and finally tried to intervene in the case management mode.Firstly,this study reviews and analyzes relevant literatures to establish the research background,purpose,significance,theoretical basis and representative research objects.Then,through a simple questionnaire survey and interview to collect the problems and needs of the rehabilitation process of the service objects,while also using the participatory observation method to understand the hemiplegia patients,and then under the guidance of the health-heart-community model,according to the collected data analysis of the common problems and needs of stroke patients.On this basis,a case management team is established to make service plans according to the multi-level needs of service objects.First of all,guided by the rational emotional therapy,using interview way of solve the problem of the service object of psychological pressure,the second joint multidisciplinary case management team on the basis of the theory of social support,provide different service object with multiple layers of other requirements,finally,the case management team for the process of the entire service intervention in patients with hemiplegia rehabilitation evaluation,reflection,finally to summarize the whole research.In general,it is hoped that the professional knowledge of social work can be used to relieve the psychological pressure of the client together with other roles of the team,and the advantages of the client can be played to help the client solve the problem,so as to explore the application of the case management mode to the rehabilitation of hemiplegia patients,hoping to produce certain reference value for future related studies.
