| Background The training of general practitioners mainly lies in the standardized training of general residents,during which the training of differential diagnosis is of great significance.However,there are few studies on the training of differential diagnosis ability in the standardized training of general practitioners.Resident case report is a kind of training to deduce diagnosis from symptoms.It is a case discussion meeting with learning as the purpose,student as the leading,and teachers and residents as participants.Objective In this study,the characteristics and advantages of Resident Case Report were discussed through questionnaire survey and test score statistics,so as to clarify its value in General practitioner training;It can also synthesize the suggestions of students to put forward optimization strategies and improvement suggestions.Methods In this study,60 students in group A(participating in resident case report)and 60 students in group B(participating in ordinary case report but not resident case report)were randomly selected from the fourth hospital of Wuhan city.The resident case report was demonstrated in both horizontal and vertical directions.The questionnaire survey,general medical science and technology ability assessment,general medical science and technology ability assessment,general medical science and technology ability assessment,general medical science and technology ability assessment and general medical science and technology ability assessment of group A and group B were conducted The results of questionnaire survey and general medical science and technology ability assessment after participating in resident case report for half a year and two years were compared.The questionnaire items include "my achievements as a speaker(group A only),the preparation time of the speaker(group A only),the gains of the participants or listeners,whether it promotes my autonomous learning,whether I feel pressure,whether I exercise general practice thinking,whether I enhance the ability of differential diagnosis,whether I enhance teaching ability,whether I exercise my speech ability,whether I exercise my psychological quality,whether I improve the ability of teaching,whether I improve the ability of speech,whether I exercise my psychological quality,whether to improve teaching ability,whether I improve my ability of speech,whether to exercise psychological quality,whether Whether to enhance reasoning ability,whether to cultivate the ability of independent thinking,whether to exercise the ability of systematic induction of knowledge,whether it is conducive to the application of knowledge in clinical practice,whether to improve the ability of inquiry,overall score,interest,depth of knowledge,degree of recommendation,quality of lectures,suggestions for resident case report(group A only)".SPSS 22.0 statistical software was used for statistical analysis.When the chi square test was used to compare the differences between group A and group B,the difference was statistically significant(P < 0.05).Results In the horizontal comparison,the number of residents case report in the following high groups was significantly higher than that in the ordinary case report(P <0.05): "overall score,whether I feel pressure,whether I promote self-learning,interest,whether I exercise the general practice thinking,whether I enhance the ability of diagnosis,whether I enhance the teaching ability,whether I exercise the ability of speech,whether I improve the ability of teaching,whether I improve the ability of speech,whether I improve the ability of self-learning,whether I improve the ability of speech,whether I improve the ability of speech,whether I improve the ability Whether they have exercised their psychological quality,strengthened their reasoning ability,cultivated their independent thinking ability,trained their ability to systematically summarize knowledge,recommended their level,and improved their interrogation ability;In "depth of knowledge,teaching quality",the number of high-level group was lower than that of ordinary case report(P <0.05).There was no significant difference between the two methods in "the gains of participants or listeners,whether it is conducive to the application of knowledge in clinical practice"(P > 0.05);The number of high resident case report group was significantly more than that of ordinary case report group(P < 0.05);For the number of medical practitioners who passed the examination at one time,more students participated in resident case report than ordinary case report(P < 0.05).In the longitudinal comparison,the average scores of resident case report after 2 years were "the average of 2 years > the average of half a year".The number of high resident case report group after 2 years of training was significantly more than that after half a year(P < 0.05);For the suggestions of trainees,it is suggested that each resident case report should last for 50 minutes,with the highest proportion(about42% of the participants suggested),and the average recommended time is 46 minutes;It is suggested that resident case report should be conducted once a week,with the highest proportion(about 60% of the total number of suggestions).In addition,students have a high support rate in "strict review of speakers,award of speakers,subsidies for participants,teacher participation and comments,and strict attendance".Conclusion Through the horizontal and vertical study of resident case report in the fourth hospital of Wuhan City,we found that resident case report has obvious advantages over ordinary case report in many aspects.The results of skill examination and the passing of medical practitioner examination also support this conclusion.It can be considered that resident case report has great value in the training of general practitioners,This method is effective and simple.It has more obvious value for new clinical doctors to train students to be familiar with the diagnosis and treatment process and develop the ability of differential diagnosis.It is worth promoting in the standardized training of general residents. |
