Objectives:Through detecting the percentage of T lymphocyte subsets CD4~+and CD8~+in peripheral venous blood of children with Henoch-Schonlein purpura(HSP)and calculating the values of CD4~+/CD8~+,to understand the relationship between them and the children with Henoch-Schonlein purpura nephritis(HSPN),explore the range of early renal damage,and provide new ideas for the prevention and treatment of HSPN.Methods:A total of 98 children with HSP treated in the Pediatric Department of the affiliated Hospital of Hebei University of Engineering from November 2019 to November 2020 were collected.According to the diagnostic criteria of HSPN,the children with HSP were divided into HSPN group(n=30)and Non-nephritic Henoch-Schonlein purpura group(n=68).And then according to whether the urine Microalbumin was higher than the normal range within one week of the course of the disease,the children with Non-nephritic Henoch-Schonlein purpura were divided into non-renal damage group(n=53)and early renal damage group(n=15).According to the clinical manifestations,the non-renal damage group can be divided into three clinical types:simple skin type,ventral type and articular type.In addition,20 healthy children who were examined in our hospital at the same stage were collected as the control group.All subjects under fasting condition were detected the values of T lymphocyte subsets CD4~+and CD8~+in peripheral venous blood by Flow Cyto Metry(FCM),and the ratio of them was calculated.Apply statistical software SPSS22.0 to analyze statistical date.P<0.05 has statistical significance.Results:1.Expression of T lymphocyte subsets in control group and three clinical types of the non-renal damage group:Compared with the control group,the levels of CD4~+and CD4~+/CD8~+in peripheral venous blood T lymphocytes of three clinical types were all decreased,while the level of CD8~+was increased.There were statistical difference(P<0.05);The levels of CD4~+,CD8~+and CD4~+/CD8~+in peripheral venous blood T lymphocytes of three clinical types:simple skin type,ventral type and articular type were no statistical difference(P>0.05).2.Expression of T lymphocyte subsets in control group,non-renal damage group,early renal damage group and HSPN group:Compared with the control group,the levels of CD4~+and CD4~+/CD8~+in peripheral venous blood T lymphocytes of non-renal damage group,early renal damage group and HSPN group were decreased,while the levels of CD8~+were increased.There were statistical difference(P<0.05);Contrasted to non-renal damage group,the levels of CD4~+and CD4~+/CD8~+in peripheral venous blood T lymphocytes of early renal damage group and HSPN group were decreased,while the levels of CD8~+were increased.The difference was statistically significant(P<0.05);Contrasted to early renal damage group,the levels of CD4~+and CD4~+/CD8~+in peripheral venous blood T lymphocytes of HSPN group was decreased,while the levels of CD8~+was increased.There were significant statistical difference(P<0.05).3.Reference range of early renal damage:interval estimation of peripheral venous blood T lymphocyte subsets CD4~+:(34.10%-36.08%),CD8~+:(32.35%-33.77%),CD4~+/CD8~+:(1.11-1.14).Conclusions:1.The changes of T lymphocyte subsets CD4~+,CD8~+and CD4~+/CD8~+were not related to the three clinical types of the non-renal damage group.2.T lymphocyte subsets CD4~+and CD8~+are involved in the pathogenesis of HSPN.The changes of T lymphocyte subsets CD4~+and CD8~+can provide a reference basis for the immune regulation of HSPN.3.The changes of T lymphocyte subsets CD4~+,CD8~+and CD4~+/CD8~+have guiding significance in the diagnosis of early renal damage in children with HSP,and can provide a reference basis for early intervention of further aggravation of renal damage and reducing the morbidity of HSPN.
