Observation On Clinical Effect Of Tianma Gouteng Decoction With Suitable Time In Treating Hypertension With Yin Deficiency And Yang Hyperactivity

Objective:To observe the clinical efficacy of Tianma Gouteng Decoction in the treatment of hypertension of Yin-deficiency and Yang-hyperactivity type,so as to provide a better therapeutic scheme for the clinical treatment of hypertension.Methods:Methods from January 2020 to January 2021,88 patients with hypertension of yin deficiency and yang hyperactivity were randomly divided into observation group and control group,44 cases in each group.Two groups of patients were treated with Tianma Gouteng drink combined with amlodipine besylate tablets,the observation group and the control group took Tianma Gouteng drink for different time.The observation group was given Chinese medicine at 8:00 and 16:00 every day,while the control group was given Chinese medicine at 30 minutes after dinner every morning for 4 weeks.Before and after treatment,TCM syndrome score,blood pressure in consulting room and 24-hour ambulatory blood pressure parameters were compared between the two groups.Result:1.The TCM syndrome scores of the two groups after treatment were significantly lower than those before treatment(P<0.05).The improvement effect of TCM syndromes in the timing medication group was better than that in the conventional medication group(P<0.05).The total effective rate of TCM syndrome in the timing medication group was 95%,and that in the conventional medication group was 87.5%,the difference was statistically significant(P<0.05).2.The clinic blood pressure of the two groups after treatment was significantly lower than that before treatment,the difference was statistically significant(P<0.05).The improvement effect of clinic blood pressure in the timing medication group was better than that in the conventional medication group(P<0.05).The total effective rate was 90% in the timing medication group and 82.5% in the routine medication group(P<0.05).3.After treatment,24hsbp,24hdbp,dsbp,ddbp,nsbp and ndbp of the two groups were significantly lower than those before treatment(P<0.05).The improvement effect of 24hsbp,dsbp,nsbp and ndbp in the timing medication group was better than that in the conventional medication group(P<0.05).There was no significant difference in 24h DBP and ddbp between the two groups after treatment(P>0.05).The total effective rate was 90% in the timing medication group and 82.5% in the routine medication group(P<0.05).The total effective rate of 24-hour average blood pressure was 95% in the timing medication group and90% in the routine medication group,and the difference was statistically significant(P<0.05).The total effective rate of daytime average blood pressure was 92.5% in the timing medication group and 90% in the routine medication group,and the difference was not statistically significant(P>0.05).The total effective rate of nocturnal mean blood pressure was 90% in the timing medication group and 67.5% in the routine medication group,and the difference was statistically significant(P<0.05).4.After treatment,the mean pulse pressure difference,mean arterial pressure,blood pressure load,nocturnal blood pressure drop rate and circadian rhythm of blood pressure of the two groups were significantly improved.The effect of mean arterial pressure,blood pressure load,nocturnal blood pressure decline rate and circadian rhythm of blood pressure in the timing group was better than that in the conventional group(P<0.05).There was no significant difference in mean pulse pressure difference between the two groups after treatment(P>0.05).There was no significant difference in blood pressure variability between the two groups before and after treatment(P>0.05).Conclusion:Taking Tianma Gouteng drink at 8:00 and 16:00,taking Tianma Gouteng drink earlier and taking Tianma Gouteng drink at 30 minutes after dinner has better effect on improving TCM syndrome,consulting room blood pressure,24hsbp,dsbp,nsbp,ndbp,mean arterial pressure,blood pressure load,nocturnal blood pressure decline rate and circadian rhythm of blood pressure.There was no obvious adverse reaction during the observation,and the safety was good.In conclusion,taking Tianma Gouteng Decoction Combined with amlodipine besylate at 8:00 and 16:00 has good clinical effect and good safety in the treatment of hypertension of yin deficiency and yang hyperactivity type.
