| Objective:（1）To investigate the status of perceived discharge preparation in patients with coronary heart disease after PCI;（2）The factors influencing the preparation of coronary heart disease patients after PCI were analyzed;（3）The influencing factors and action path of the preparation for discharge after PCI were analyzed by structural equation model.Methods:In this study,430 patients with coronary heart disease after PCI in Department of cardiovascular medicine from July to December 2020 in two tertiary first-class hospitals in Yan’an City were selected as the research objects.Convenient sampling method was used to conduct a questionnaire survey,including general information,discharge readiness and medical coping style,social support,self-efficacy,discharge guidance quality,etc.The effective questionnaire information was entered by epidata3.1 double person,and spss25.0 and amos24.0 software were used for statistical analysis.The analysis method includes statistical description（mean±Standard deviation,frequency,composition ratio,statistical inference（single factor analysis）and correlation analysis.The model of structural equation is constructed by taking medical response,social support,self-efficacy and discharge guidance quality as independent variables,and the preparation for discharge as dependent variable.The path analysis of influencing factors is carried out based on structural equation model.Result:1.Patients with coronary heart disease had an average score of（80.13±8.47）points for their perceived readiness for discharge after PCI;2.In terms of general sociodemographic characteristics and disease-related conditions,univariate analysis shows that the readiness for discharge of patients with coronary heart disease after PCI is related to gender,education level,work status,family monthly income,perceived self-care ability,and other types of diseases.The difference is statistically significant（P<0.01）;3.Patients with coronary heart disease who underwent PCI had an average score of（16.96±3.13）points for facing the disease,an average score for avoidance（14.63±3.21）,an average score for yielding methods（9.59±3.53）,facing methods and yielding methods Compared with the domestic norm,the difference is statistically significant（P<0.01）,and the scores are all lower than the domestic norm;4.The average score of social support in patients with coronary heart disease after PCI is（32.71±5.15）,the average score of objective support（9.07±1.54）,the average score of subjective support（17.71±3.46）,and the average score of social support utilization（5.93）±1.64）points,patients with coronary heart disease who underwent PCI as a whole belong to a moderate degree of social support;5.The average score of self-efficacy in patients with coronary heart disease after PCI is（39.17±6.38）,the average score of management disease symptom dimension is（24.46±4.70）,the average score of management disease common problem dimension is（14.70±2.32）,self-efficacy The overall level is moderate;6.The average score of discharge guidance quality of patients with coronary heart disease after PCI is（107.70±8.85）points,the average score of dimension acquisition content is（34.66±3.49）points,and the average score of skill and effect of guidance content is（75.04±6.43）points,showing the overall quality of discharge guidance was low.7.The total score of discharge readiness in patients with coronary heart disease after PCI is negatively correlated with avoidance style and yield style(r avoid=-0.549,r yield=-0.650,both P<0.01);It is related to facing style,social support,and self-efficacy the quality of discharge guidance was positively correlated(r face=0.537,r social=0.414,r Self-efficacy=0.742,r Discharge=0.655,both P<0.01).8.Taking disease coping styles as independent variables,social support,self-efficacy,and discharge guidance quality as intermediary variables,and discharge readiness as dependent variables,a structural equation model of influencing factors of coronary heart disease PCI operation discharge readiness is established,which can explain discharge readiness the variance reached 63%.Conclusion:1.The preparation for discharge of patients with coronary heart disease after PCI is at a moderate level.2.The factors that affect the readiness of patients with coronary heart disease after PCI are:gender,education level,occupational status,family income,perceived self-care ability,and other types of diseases.3.The readiness for discharge of patients with coronary heart disease after PCI is positively correlated with facing style,social support,self-efficacy,and quality of discharge guidance;It is negatively correlated with avoidance style and yield style.4.The structural equation model shows that:face style,avoidance style,and social support have an indirect effect on discharge readiness;The quality of discharge guidance has a direct effect on discharge readiness;Yielding style and self-efficacy have both direct and significant influence on discharge readiness Indirect effects;Self-efficacy has the largest total effect size on discharge readiness through direct effect size and indirect effect size. |
