| It’s been more than 10 years since the initiation of new medical reform.As an important measure for the country to promote the hierarchical diagnosis and treatment system,the construction of medical consortia is a key deployment for promoting the sinking of high-quality resources and optimizing resource allocation.During the development progress of pushing forward our country’s health resources to be distributed more evenly and accessibly,the cross-regional medical consortium is the key carrier.By understanding the common problems and dilemmas in the construction and promotion of the cross-regional medical consortium,to adapt to the deepening of medical reform,promoting hierarchical diagnosis treatment,integrating medical resources,and founding of cross-region medical cooperation development long-term mechanism.This paper is based on above research background.On the basis of combing related theories such as incomplete contract theory,system theory,transaction cost,etc.we analysis real cases of cross-region hospital medical consortium construction,and we find out several problems,such as no obvious effects in resources integration,immaturity of Cooperate Governance structure,incomplete internal coordinate mechanism,and lack of consistency of synergic effects,etc.Combined expert interviews and theory analysis,we find out three primary causes which resulted in the above problems: geographical limitations,unclear dominance/division of the government,and incomplete incentive/constraint mechanism.In order to realize the sustainable development of cross-region hospital medical consortium and conquer the cooperation dilemmas,we should take full consideration of the consortium’s integration,correlation,dynamics,and have a full understanding of consortium’s existence and evolution law.Meanwhile,we also need to take into account factors such as information asymmetry between different members of the consortium;policy and environment need to be optimized,incomplete contract,etc.Based on above circumstances,we make several proposes such as: strengthen organization leadership;fulfill main body responsibility,and intensify the cooperation of different departments,so as to full play the superimposed effect.We can also build farm-out mechanism;perfect the incentive/constraint system;enhance the information construction,to break communication barriers;have a full control of the consortium’s existence/evolution law and adjust policies and guidelines dynamically. |
