| Objective: To explore the risk factors of postoperative pulmonary complications(PPCs)after radical resection of lung cancer in elderly patients,explore the value of lung ultrasound score(LUS)in evaluating of PPCs.Methods: Select 270 elderly patients with lung cancer who underwent thoracoscopic lobectomy from July 2019 to December 2020,the clinical data including age,ASA grade and so on were collected and analyzed.lung ultrasound examination was performed preoperatively to calculate LUS.Record the occurrence of PPCs,and patients were divided into: PPC group(with PPCs)and n PPC group(without PPCs).Logistic regression analysis incorporated factors with P<0.10 in univariate analysis,to identify the risk factors for PPCs.Finally,we used ROC curve to assess the predictive effect of LUS in patients with PPCs.Results: The incidence of PPCs was 38.5%.Age,COPD,anemia and LUS in PPC group were statistically significant higher than those in n PPC group(P<0.05),Logistic regression analysis showed they were all risk factors of PPCs(P<0.05).LUS(14.04±3.42)in PPC group was higher than that in n PPC group(10.42±3.34)(P<0.001).The area under ROC curve is: 0.777,the best cut-off value of LUS between PPC group and n PPC group is 13.5 points.At this time,the sensitivity of LUS in predicting PPCs was 65.4%,specificity was 79.5%.Conclusion: Age,COPD,anemia and LUS were independent risk factors for PPCs.Patients with LUS>13 points have a higher risk of PPCs,more attention should be paid to them and more effective prevention strategies should be actively developed. |
