| Objective: To investigate the association between TRADD and PTMC by detecting levels of serum TRADD in patients with papillary thyroid microcarcinoma(PTMC),and to provide a diagnostic tool and a potential therapeutic target for the diagnosis and screening of high-risk PTMC.Methods: Among the patients with thyroid nodule admitted to the Department of Head and Neck Surgery of Gansu Cancer Hospital and Gansu Provincial People’s Hospital were enrolled from June 2018 to June 2019,We selected a total of 144 patients who met the criteria according to the postoperative pathologic results will be divided into these patients with thyroid nodules of thyroid papillary carcinoma group(PTMC)73cases.There were 71 patients in the nodular goiter group.In addition,83 healthy volunteers were selected as the healthy control group in the Physical Examination Center of Gansu Provincial People’s Hospital.We collect the thyroid nodule patients and healthy volunteers of medical data in detail,doing the thyroid gland colour to exceed examination,testing thyroid function(TSH,T3,T4,A-Tg,A-TPo)electricity chemiluminescence method and detection of serum TRADD levels with ELISA method for the people to compare the correlation between serum TRADD expression level and thyroid micropapillary carcinoma group(PTMC).Results: The level of serum TRADD expression in PTMC group was significantly higher than that in nodular goiter group and healthy control group(P < 0.001).Spearman correlation analysis showed that serum TRADD level was significantly positively correlated with properties,Ti-RADS,T3,T4,TSH,A-TG and A-TPO.There was A significant positive correlation between serum TRADD,TI-RADS,T3,T4,TSH,A-TG and A-TPO.Multiple linear regression analysis of serum TRADD showed that nature,age and T3 were independent influencing factors of TRADD.LGistic logistic regression analysis of PTMC showed that serum TRADD(OR=1.007,95%CI:1.002-1.013,P=0.004)and TI-RADS(OR=92.337,95%CI:18.295-466.048,P < 0.001)were the risk factors for PTMC.The results of the graph of the efficacy of PTMC predicted by TRADD were as follows: using serum TRADD as the test variable and diagnostic PTMC as the status variable to do the ROC curve,the optimal truncation value of TRADD for the diagnosis of PTMC was 658.005 pg/ml,the sensitivity was 86.3%,the specificity was 74.7%,and the area under the curve was 0.892(95%CI: 0.852--0.933,P < 0.001).The correlation between TRADD and PTMC clinicopathology showed that the serum TRADD expression level of PTMC patients was significantly correlated with capsule invasion(P<0.05),but no significant correlation with gender,age,tumor diameter,lymph node metastasis,and clinical stage(P > 0.05).Spearman correlation analysis of PTMC membrane invasion showed that there was a significant positive correlation between membrane invasion and serum TRADD(r=0.352,P=0.002).There was no significant correlation between TI-RADS,T3,T4,TSH,A-TG and A-TPO.LGistic logistic regression analysis showed that serum TRADD(OR=1.006,95%CI:1.000-1.011,P=0.014)was a risk factor for PTMC membrane invasion.Using serum TRADD as the test variable and PTMC invasion as the status variable,the ROC curve was performed.The optimal cut-off value of TRADD for PTMC invasion was 860.455 pg /ml,the sensitivity was 61.1%,the specificity was 84.2%,and the area under the curve was 0.730(95%CI: 0.590--0.870,P=0.003).Conclusions: TRADD serum levels will increase of thyroid papillary carcinoma(PTMC)risk and can increase the risk of capsule invasion and specificity of the serum level of TRADD may be tiny thyroid papillary carcinoma(PTMC)a new biological marker of its pathogenesis needs further research. |
