| Purpose:The purpose of this paper is to do two aspects of research.First,to investigate the current situation of self-neglect among hospitalized older adults and to explore and analyze its influencing factors.Secondly,to develop an intervention program for self-neglect.According to the intervention program,the intervention group was helped to self-neglect the hospitalized elderly,and the quantitative analysis was conducted with the use of a scale to test the effectiveness of the intervention,so as to provide a reference basis for the home care of the hospitalized elderly and their caregivers.Methods:(1)Survey study:Using convenience sampling method,320 hospitalized elderly people in Linyi city were selected as the study subjects.We recovered a total of 318 valid questionnaires,including their general information and self-neglect survey.Descriptive statistics,non-parametric test of independent samples,logistic regression analysis and other statistical methods were used to understand the current situation of self-neglect among hospitalized elderly and analyze its influencing factors.(2)Intervention study: We divided 60 self-neglected hospitalized older adults into an intervention group and a control group.Older adults in the intervention group will undergo a 6-month intervention activity.The survey will use general information,self-neglect scale,depression scale,and social support scale,followed by statistical processing using paired sample t-test and independent sample t-test,and then compare and analyze the indicators of the two groups before and after the intervention.Result:1.Research and study(1)The score of self-neglect in hospitalized older adults was 11(5,15),and the prevalence of self-neglect was 27.7%.(2)The scores of each dimension of self-neglect among hospitalized older adults were medical 32(0.75,4),health 3(0,4),emotional 32(1,4),safety 1.5(0,4),and social interaction1(0,3),with the highest score being health self-neglect and the lowest being social interaction self-neglect.(3)Advanced age,male,perceived financial stress,not in marriage,living alone,and low literacy were risk factors for self-neglect(p<0.05).2.Intervention study(1)The paired t-test before and after the intervention showed that the difference in total self-neglect scores before and after the intervention was statistically significant for the older adults in the intervention group,while the difference in scores before and after was not statistically significant for the older adults in the control group.The difference between the pre and post scores of the two groups showed that the total score of self-neglect in the intervention group(t=-3.282,p=0.002)was significantly lower than that in the control group,and the difference was statistically significant.(2)Paired non-parametric tests before and after the intervention showed that the changes in the pre-and post-scores of the medical,health,emotional,safety,and social interaction dimensions of self-neglect among the elderly in the intervention group were statistically significant,while the differences in the scores of the self-neglect dimensions among the elderly in the control group were not statistically significant.Comparing the scores of the dimensions of self-neglect between the two groups of older adults showed that the intervention group in medical(z=-3.579 b,p=0.000),health(z=-4.529 b,p=0.000),emotional(z=-3.116 b,p=0.002),safety(z=-2.749 b,p=0.006),and social interaction dimensions(z=-3.500 b,p= 0.000)scores were significantly lower than the control group and the differences were statistically significant.(3)Paired t-test before and after the intervention showed that the difference in total depression scores before and after the intervention was statistically significant in the self-neglected elderly in the intervention group,while the difference in total depression scores in the elderly in the control group was not statistically significant.Comparing the difference between the scores of the two groups before and after the intervention showed that the depression scores of the elderly in the intervention group(t=-2.41,p=0.019)were significantly lower compared to the control group,and the difference was statistically significant.Conclusion:(1)Self-neglect is more common among hospitalized older adults,but the overall level of social awareness of it is low,and society does not pay extensive attention to the problem of self-neglect.Relevant medical departments should take appropriate measures to strengthen prevention and intervention to promote the physical and mental health of hospitalized older adults.(2)Multidisciplinary team interventions can reduce the level of self-neglect in hospitalized older adults,alleviate depressive symptoms in elderly self-neglect patients,and improve the quality of life of self-neglect older adults,and be applied to the daily care of hospitalized older adults. |
