| Objective:To discuss the clinical value of temporary block of laparoscopic uterine artery +lesions resection for the treatment of exoded uterine scar pregnancy after caesarean section. Methods:Collected 104 patients with exogenous cesarean scar pregnancy as the research objects from 5 obstetrics and gynecology of hospitals(Jiangxi Provincial People’s Hospital,Shangrao Municipal People’s Hospital,Jingdezhen First People’s Hospital,Jiujiang Maternity and Child Health Hospital,Shangrao Health School Affiliated Hospital)from July 2016 to June 2018 who were required to reproduce Pregnancy.Randomly divided into 3 groups.Among them,Laparoscopic uterine artery temporary occlusion + lesion resection in 33 cases(group A);Laparoscopic direct lesion resection in 35 cases(group B);uterine artery embolization + Laparoscopic lesion resection in 36 cases(group C).The intraoperative blood loss, intraoperative and postoperative blood transfusion rate, operation time, blood β-HCG return to normal time, postoperative hospital stays, postoperative pain index and postoperative pregnancy rate were compared among the three groups.Results:1.All three groups completed the operation successfully. There was no significant difference in operation time, blood β-HCG return to normal time, and hospitalization time in each group(P>0.05);2.The intraoperative blood loss and intraoperative and postoperative blood transfusion rate of group A was less than those of group B,and the difference was significant(P<0.05),while the difference between group A and group C was not significant(P>0.05);3.The pain index VAS scores of group A was lower than group C at 24 h and 48 h after operation, and the difference was significant(P<0.05),while the difference between group A and group B was not significant(P>0.05);4.The pregnancy rate of group A from 1 year to 2 years after operation was significantly higher than that of group C,the difference was significant(P<0.05),but the difference between group A and B was not significant(P>0.05)).Conclusion:Temporary block of laparoscopic uterine artery + lesion resection for the treatment of uterine scar pregnancy after exogenous cesarean section has definite curative effect, less bleeding, little influence on re-pregnancy after surgery, and has a good clinical application. |
