| Objective:The purpose of this study is to explore the diagnosis,surgical indication,surgical treatment and curative effect of benign liver tumor.Method:This study retrospectively analyzed the clinical data of 173 patients with benign liver tumors who underwent surgical treatment in the Department of Hepatobiliary Surgery of the First Affiliated Hospital of Nanchang University from April 2014 to August 2020.There were 136 cases of cavernous hemangioma,19 cases of focal nodular hyperplasia,7 cases of angiomyolipoma,5 cases of epithelioid angiomyolipoma,4 cases of hepatic adenoma,and 2 cases of inflammatory granuloma.Result:Imaging examinations were performed in 173 patients,and the overall diagnostic coincidence rate was 86.13%.All patients met the clinical guidelines for surgical indications,including 73 patients in the Laparoscopic hepatectomy(LH)group and100 in the Open hepatectomy group.There was no statistical difference in preoperative data between the two groups.There were no significant differences in the amount of intraoperative blood loss,operative time,intraoperative blood transfusion rate and postoperative complications between the two groups(P>0.05).The portal occlusion rate of laparoscopic group was higher than that of laparoscopic group(P=0.03),and the postoperative hospital stay of laparoscopic group was less than that of laparoscopic group(P < 0.01).Three patients in the LH group were transferred to open surgery.The levels of ALT and AST in the LH group were lower than those in the OH group on the first and second days after surgery,and the difference was statistically significant(P < 0.05).In the difficult group(group A),the amount of surgical blood loss and intraoperative blood transfusion rate in the LH group were higher than those in the OH group(P < 0.05),except for the first and second days after surgery,the decrease of hemoglobin in the LH group was significantly lower than that in the OH group,the difference was statistically significant(P < 0.05).Other data differences were not statistically significant.In the tumor diameter ≥10cm group(group B),the increase of AST and direct bilirubin in the LH group on the first day after surgery was significantly lower than that in the OH group(P < 0.05),and the decrease of hemoglobin on the first and second day after surgery was lower than that in the OH group,with statistically significant differences(P < 0.05),while the other data showed no statistically significant differences.Conclusion:1.The diagnosis of benign liver tumors depends on imaging,which should be fully combined with the medical history;2.Surgical resection is the most commonly used treatment method,but the surgical indications should be strictly grasped.Both open hepatectomy and laparoscopic hepatectomy are safe and effective methods.3.In the difficult group,both the laparoscopic group and the laparoscopic group were safe and effective,although the laparoscopic group had more bleeding.In the tumor ≥ 10 cm group,both the open hepatectomy group and the laparoscopic hepatectomy group were safe and effective.Individual treatment options should be made according to the specific conditions of the patients to improve the level of surgical diagnosis and treatment of benign liver tumors... |
