Purpose:The preoperative serum levels of CA 125,CA 199,CEA and AFP were used to analyze their correlation with postoperative pathological features and ovarian metastasis in patients with cervical adenocarcinoma,so as to provide clinical reference for patients with cervical adenocarcinoma to retain their ovaries.Methods:A retrospective analysis of patients who underwent extensive hysterectomy at the First Affiliated Hospital of Nanchang University from January 1,2015 to December 31,2020,and were diagnosed with cervical adenocarcinoma(common type)through postoperative pathology,and were of primary cervix.Analyze the correlation between preoperative tumor marker levels and clinicopathological characteristics and ovarian metastasis.Results:A total of 87 cases of patients with common cervical adenocarcinoma were included in this study,of which 18 cases(20.7%)increased serum CA 125,15 cases(17.2%)increased serum CA 199,and 10 cases(11.5%)increased serum CEA.And 4cases(4.6%)increased serum AFP;11 cases had both serum tumor markers increased at the same time before surgery,of which 7 cases(8.0%)increased both serum CA125 and CA 199,CA 125 and CEA increased at the same time 2 cases(2.3%)were high,CA 199 and CEA were both elevated in 1 case(1.1%),and serum CEA and AFP were both elevated in 1 case(1.1%);3 serum tumor markers were elevated in 2 cases before surgery,Serum CA 125,CA 199,and CEA increased in 1 case(1.1%),and serum CA 125,CA 199,and AFP increased in 1 case(1.1%).Postoperative pathological features included 11 cases(12.6%)with tumor diameter> 4cm,18 cases(20.7%)with lymphatic vascular invasion,45 cases(51.7%)with deep cervical interstitial invasion,12 cases(13.8%)with uterine body involvement,lymph nodes There were 15 cases(17.2%)with metastasis,of which 2 cases had para-aortic lymph node metastasis,3 cases(3.4%)with parauterine infiltration,2 cases(2.3%)with positive margins,69 cases(79.3%)with FIGO stage I-IIA)(13 cases of stage IA,51 cases of stage IB,5 cases of stage IIA),18 cases(20.7%)of stage IIB-III(3 cases of stage IIB,15 cases of stage IIIC),71 cases of bilateral ovariectomy(81.6 %),3 cases of ovarian metastasis(3.4%),unilateral oophorectomy and contralateral ovarian biopsy in 2 cases(2.3%),bilateral ovarian biopsy in 14 cases(16.1%),no ovarian metastasis was found in rapid intraoperative pathology.Increased serum CA 125 level is significantly correlated with late FIGO staging,deep cervical interstitial infiltration,uterine body involvement,lymph node metastasis,tumor diameter> 4cm;increased serum CA 199 level is associated with late FIGO staging,deep cervical interstitial infiltration,uterine body Involvement and lymph node metastasis are significantly related.The simultaneous increase of serum CA 125 and CA 199 levels is significantly related to late FIGO staging,deep cervical interstitial infiltration,uterine corpus involvement,and lymph node metastasis.Multivariate logistic regression analysis of the significantly related factors in univariate analysis showed that the preoperative increase in serum CA125 was an independent risk factor for uterine corpus involvement(P<0.05),and the simultaneous increase of preoperative serum CA125 and CA 199 was the uterine corpus involvement.Independent risk factors(P<0.05).Univariate analysis showed that uterine body involvement was significantly related to ovarian metastasis.Conclusions:1.The increase of serum CA 125 and CA 199 levels is significantly correlated with postoperative pathological characteristics;2.Elevated serum CA 125 is an independent risk factor for uterine body involvement;3.The simultaneous increase of serum CA 125 and CA 199 is an independent risk factor for uterine body involvement;4.Uterine body involvement is significantly related to ovarian metastasis.
