| Objective:To explore the influence of hospital simulation training combined with WeChat visit nursing intervention on the incidence of stoma complications,stoma patients’ self-care ability and quality of life,and to provide a new practical method for improving patients’ self-care ability and quality of life.Methods:In this study,a total of 124 enterostomy patients who met the inclusion criteria in the gastrointestinal surgery department of a third-grade grade A general hospital in Jiangxi Province from June 2019 to May 2020 were included.They were divided into control group and intervention group by random number table method,with 62 patients in each group.The control group was treated with routine ostomy nursing measures,while the intervention group was given standardized simulation training on routine nursing intervention for patients with ostomy,and regularly visited patients on WeChat.The incidence of stoma complications was observed 15 days after discharge,1 month after discharge and 3 months after discharge.The self-care ability and stoma quality of life of the patients were investigated.To analyze and evaluate the effects of in-hospital simulation training combined with WeChat visit on the quality of life of patients with enterostomy.Results:1.Comparison of the incidence of enterostomy complications between the two groupsA total of 64 cases(17.8%)of stoma complications occurred in the two groups.The highest incidence of stomatal complications was observed 1 month after discharge.The incidence of ostomy complications in the intervention group at 15 days,1 month and 3 months after discharge(13.3%,17.6% and 8.3%)was lower than that in the control group(16.6%,31.7% and 21.7%).The X2 values of the control group and the intervention group were 0.263(P=0.637),4.663(P=0.021)and 4.182(P=0.036),respectively,indicating that there was no statistical difference in the incidence of complications between the two groups on the 15 th day after discharge,while there was statistical significance on the 1st and 3rd month after discharge.2.Comparison of ostomy self-care abilityComparing the self-care ability of patients in the two groups after stomy surgery,it was found that there were statistically significant differences between the intervention group and the control group in self-care ability,self-care knowledge,self-care skills and other scores 15 days,1 month and 3 months after discharge from enterostomy(P <0.05).There were no significant differences in self-care intention and nursing knowledge scores in the control group at 15 days,1 month and 3 months after discharge,while nursing skills improved.Repeated measurement ANOVA was conducted for the two groups of data.The results showed that the time-point effect could significantly affect the change of self-care ability,F(2,57)=40.934,P<0.01,meaning that the value of self-care ability of patients would change at any time.Secondly,the self-care ability was significantly affected by the treatment methods,F(1,58)=15.200,P<0.01;Furthermore,there was a significant interaction effect between time point and intervention mode,F(2,57)=7.429,P<0.05,indicating that the changing trend of self-care ability with time point would be different due to different intervention mode.3.Comparison of quality of life in enterostomyThe comparison of postoperative quality of life between the two groups showed that there were statistically significant differences in physical,psychological,social and mental health scores between the intervention group and the control group 1month and 3 months after discharge(P <0.05).After 15 days of discharge,there was no significant difference in physiology between the control group and the intervention group(P > 0.05).In the control group,there were statistical differences in physiological,psychological and social indicators at 15 days,1 month and 3 months after discharge,with time effect,while in the intervention group,one-way analysis of variance also showed statistical significance(P <0.05).Repeated measure ANOVA in general linear model was used to analyze the quality of life data of the two groups.Time-point effect can significantly affect the change of quality of life(F(2,57)=54.387,P<0.01),which means that the quality of life of patients will change at any time.Secondly,the intervention mode could significantly affect the quality of life(F(1,58)=13.724,P<0.01);Furthermore,there was a significant interaction effect between time point and intervention mode,F(2,57)=20.062,P<0.01,indicating that the changing trend of self-care ability with time point would be different due to different intervention mode.Conclusion:Simulation training in hospital can effectively reduce the incidence of stomy complications,improve the self-care ability of enterostomy patients,and improve the quality of life of patients. |
