ObjectiveTo investigate the clinical characteristics and pathological types of young lung cancer patients(age ≤45 years)and the prognosis of young extensive small cell lung cancer,so as to further understand the characteristics of young lung cancer.MethodsA total of 125 young lung cancer patients who met the admission and discharge criteria of this study were collected from January 2014 to October2020(Seven patients of Tu,Salar,Mongolian,Manchu, large cell carcinoma and carcinoid carcinoma were not included in the analysis due to the small number of cases).Patients were retrospectively analyzed for gender, age, smoking history, ethnic origin, tumor family history, first symptom, tumor location, carcinoembryonic antigen(CEA),carbohydrate antigen 125(CA125),pathological type,TNM stage, treatment modality, etc. The clinical features of young lung cancer were compared, and the progression-free survival(PFS)of SCLC patients with complete data were analyzed.ResultsIn this study, young lung cancer patients were collected between the ages of25 and 45 years old, with the majority of patients aged between 36 and 45 years old. There was no significant difference in smoking status among young lung adenocarcinoma, squamous cell carcinoma and small cell lung cancer(P=0.732).Adenocarcinoma accounted for a large proportion in the Han,Tibetan and Hui ethnic groups, while squamous cell carcinoma and small cell carcinoma accounted for a small proportion, but there was no statistically significant difference(P=0.193).There were statistically significant differences in the first visit symptoms among different pathological types of young lung cancer(P=0.016).The proportion of serum CEA>5ng/ml and CA125>35U/ml in young lung adenocarcinoma patients was higher than that in squamous cell carcinoma and small cell carcinoma, and the difference was statistically significant(P<0.05).Extensive period of youth small cell lung cancer of the median progression-free surial was 3.9 months, single factor analysis showed that chemotherapy cycles has correlation with progression-free survival, sex, smoking history,ECOG score, treatment, chemotherapy regimens,CEA level at diagnosis and NSE level at diagnosis has nothing to do with progression-free surial, cycles of chemotherapy is a broad stage small cell lung cancer affects young progression-free surial of independent factors(P=0.000).ConclusionsMale is the most common lung cancer among young people in Qinghai. The first symptoms were cough and sputum.Adenocarcinoma was the most common pathological type.Diagnosis is usually advanced.Brain metastasis was more common in young lung cancer. Most patients were treated with chemotherapy alone. The number of chemotherapy cycles is an independent factor affecting the progression-free survival of young adults with extensive small-cell lung cancer.
