With the development of information technology,especially the popularization of medical informatization,medical informatization technology with digital medical records as an entry point is not only an important foundation for medical information sharing,but also a basic data source for building a value society.However,how to share medical information more effectively is still an issue that needs to be further optimized.Especially as the hierarchical diagnosis and treatment mechanism advances,exploring a medical data sharing method that can better fit the effective implementation of the hierarchical diagnosis and treatment mechanism is still an open research direction.According to the data analysis of my country’s medical information construction in recent years,digital medical record data sharing is not popular,and there are the following problems: 1)The degree of trust caused by centralized management is low.A single storage and backup point has a greater chance of data leakage and tampering.All the medical record data of the user is hosted in the hospital for management,which is easy to cause an unequal state of rights.Medical institutions are in their own right,and there are issues of absolute trust in themselves and distrust of other medical institutions,resulting in negative feedback of data sharing.2)Lack of reasonable confirmation of power and information retrospective protection mechanism.The operation of the entire access phase lacks a trusted public platform to verify all processes,there is no reasonable mechanism to confirm the ownership of responsibility,and the rationality of the distribution of permissions is also doubtful.Therefore,in the context of the hierarchical diagnosis and treatment system,this thesis applies blockchain as a technology platform to digital medical record sharing,and conducts in-depth research on the blockchain-based digital medical record sharing method around application scenarios and actual business characteristics.The main work is as follows:(1)Through the analysis of the storage and sharing mechanism of digital medical records in the existing hospital information system,as well as the business characteristics,in-depth exploration of the compatibility of the mainstream consensus algorithm in the current blockchain technology with the hierarchical diagnosis and treatment mechanism,and according to the classification The special node weights and architecture features in diagnosis and treatment have proposed a hierarchical consensus algorithm that meets the current digital medical record sharing needs.Among them,each node is assigned a different weight WEIGHT when it enters the network.In the committee election and reconfiguration process,the verifiable random functions VRF and WEIGHT are introduced,and the VFR-based Po S consensus algorithm is used to set up to solve the opponent’s bias in the reconfiguration process.problem.Through the hierarchical design of the committee member election process to execute transactions in parallel,so as to improve transaction speed and transaction throughput.Finally,the feasibility of the algorithm is verified through experimental tests.(2)By analyzing the current deficiencies of typical hospital information systems and platforms in sharing digital medical records,combined with a threshold secret sharing scheme,a medical record data sharing method DP-MDSM under the background of hierarchical diagnosis and treatment is proposed,and the initialization phase is introduced in detail.,The specific workflow of the request access phase and the verification phase.On this basis,in view of the security issues of data sharing,different transaction formats are designed,and the asymmetric cryptographic method is used to make the medical record data on the blockchain in ciphertext.At the same time,in order to reduce the verification burden of users and server nodes,a blockchain verification node is set up to undertake some verifiable calculations.Finally,the comparative analysis shows that this method improves the average time consumption of transaction verification and meets the processing speed of the blockchain platform.(3)Based on the above consensus algorithm and sharing method,the overall framework of the digital medical record model is designed,and on this basis,the overall design and prototype implementation of the platform are completed in conjunction with the fabric blockchain application architecture.
