Analysis On The Operating Status And Countermeasures And Suggestions Of Drug Volume-based Purchasing Policy In Jiangxi Province From The Perspective Of Suppliers

Objective:From the perspective of drug suppliers,to grasp the implementation status of the drug Volume-Based Purchasing Policy in Jiangxi Province,find out the problems existing in the operation of the drug Volume-Based Purchasing Policy,and put forward corresponding countermeasures and measures.It provides a reference basis for improving the drug Volume-Based Purchasing Policy,and provides experience for the development of drug Volume-Based Purchasing Policy in other areas.Methods:Self-made questionnaire was used to measure reliability and validity.A total of539 cases of drug tendering personnel in drug supply enterprises were randomly selected from August to December 2020.The investigation and interview on the current situation of drug Volume-Based Purchasing Policy in Jiangxi Province were carried out.Intrinsic consistency test and exploratory factor analysis were used to analyze the reliability and validity of the questionnaire;The qualitative cognitive data of the operation of the policy were analyzed by fishbone diagram method,and the quantitative data were statistically analyzed by t test and analysis of variance.Result:(1)After the screening of the questionnaire on the status quo of the operation of the drug Volume-Based Purchasing Policy,It involves five dimensions of policy connotation,policy benefit,enterprise profit,Policies benefiting the people and competitive pressure,with 23 items.(1)The reliability measurement showed that the overall Cronbach’s α value was 0.917,and the Cronbach’s α values of the five dimensions were respectively 0.917、0.930、0.967、0.768、0.873、0.568.(2)Validity measurement found that the KMO value was 0.863,And Bartelett’s spherical test was statistically significant(P<0.001),the extracted 5 common factors are consistent with the design dimension,the factor loads after rotation are respectively 31.7%、19.8%、10.4%、10.1%、8.8%,the cumulative contribution rate reaches 80.764%.The 23 items rotated component matrix corresponds to 5 common factors.(3)The evaluation results of different groups have good consistency.(2)Quantitative results showed that: the general evaluation of the drug Volume-Based Purchasing Policy by the staff of the drug supply enterprise is divided into 3.53?0.40 points,the scores of policy connotation,policy benefit,enterprise benefit,Policies benefiting the people and competitive pressure are as follows:3.75?0.56 points、3.59?0.63 points、2.25?0.73 points、4.07?0.59 points、3.68?0.61 points,there were differences in scores among different dimensions(P<0.001),among them,the Policies benefiting the people dimension is the highest,the enterprise income is the lowest.(1)Among the five items of policy connotation dimension,the necessity score of policy expansion was the lowest 3.34?0.62 points;(2)Among the 9 items of policy benefit,the score of improving drug quality was the lowest 3.27?0.96 points;The second is to reduce the cost of the enterprise 3.50?0.93 points.(3)Among the three items of enterprise income,the profit score of the unsuccessful enterprise is the lowest 2.17?0.98 points;(4)In the three items of Policies benefiting the people to the people,the people got the lowest score 3.91?0.84points;(5)Among the three items of competitive pressure,the score of intensifying malignant competition was the lowest 3.60?0.91 points.(3)Qualitative results showed that:(1)The main policy problem in the enterprise income dimension is the lack of "control measures for bad market competition behavior" and "effective protection measures for small and medium-sized pharmaceutical enterprises".(2)The main policy problem in the dimension of policy benefit is the lack of "the guarantee policy that drug quality does not decline" and "the enterprise cost accounting control measures".(3)In terms of enterprise competition pressure,the policy lacks "drug shortage emergency system","control measures for monopolistic and other evil competition behaviors" and "differentiated policies for different types of enterprises of large,medium and small size to carry out procurement with quantity".(4)In terms of policy connotation,the policy lacks "procurement publicity and implementation system with quantity","accurate budget forecast model of procurement quantity" and "planning policy for expanding the scope of included varieties".(6)From the perspective of Policies benefiting the people,the "individualized medication demand guarantee mechanism" should be perfected.Conclusion:The self-made questionnaire with 5 dimensions and 23 items for the operation of drug Volume-Based Purchasing Policy had good reliability and validity.The quantitative results were consistent with the qualitative results.It was found that the personnel of related enterprises generally recognized the drug Volume-Based Purchasing Policy,The key point is to strengthen the guarantee policy of enterprise income dimension;It is advisable to strengthen the regulation of bad market competition behavior effective protection for small and medium-sized pharmaceutical enterprises、guarantee that the quality of medicines does not decline、drug shortage emergency response system、different types of enterprises are classified to purchase in quantity、procurement policy of drugs with quantity is publicized and implemented、supporting policies such as accurate prediction of purchase quantity,planning and promotion of expanding the scope of drug varieties included,and guarantee of patients’ personalized medication needs.In order to ensure the healthy and orderly development of drug supply market,promote the modernization of drug procurement management system and the improvement of governance ability.
