BackgroundAspirin,with good antiplatelet effects,has been used among patients with recurrent cerebral infarction(RCI),but the patients’ response to it varies greatly.The ischemia happens when some can not inhibit platelet aggregation effectively after taking aspirin.This phenomenon,called aspirin resistance(AR),is closely linked to RCI.The mechanism of AR in patients with RCI is unknown,but multiple factors are involved including gene polymorphism.To clarify the incidence and risk factors of AR is one of the urgent problems faced by neurologists.ObjectiveThe purpose of this study is to investigate the incidence of AR in patients with RCI,as well as explore the high-risk factors for AR and the relationship between PEARI gene polymorphism and AR.Methods80 patients with RCT,treated in the Department of Neurology of the First People’s Hospital of Shangqiu City from January to December 2020,were selected to detect the content of serum thromboxane B2(TXB2).The participants were divided into two groups,aspirin resistance(AR)group and aspirin sensitive(AS)group.The minimum concentration of TXB2 in AR group was more than3.1ng/ml.The genotypes of PEAR1 rs12041331,rs12566888 and rs2644592 were determined by high throughput and first generation sequencing.Results1.We found that the prevalence rate of AR in the studied population(23 cases)was 28.75%.2.Genotype distribution of PEAR1 rs12041331 in AR group was 6 cases of GG genotype(26.10%),13 cases of GA genotype(56.5%)and 4 cases of AA genotype(17.4%).The genotype distribution of PEAR1 rs12041331 in AS group was 30 cases of GG genotype(52.6%),22 cases of GA genotype(38.60%)and 5 cases of AA genotype(8.8%).The distribution of PEAR1 rs12566888 genotype in AR group was 2 cases of GG genotype in(8.7%),15 cases of GT genotype(65.20%),6cases of TT genotype(26.10%).The genotype distribution of PEAR1 rs12566888 in AS group was 30 cases of GG genotype(52.60%),21 cases of GT genotype(36.80%),and 6 cases of TT genotype(10.50%).The genotype distribution of PEAR1 rs2644592 in AR group was 2 cases of GG genotype(8.70%),11 cases of GA genotype(47.80%),10 cases of AA genotype(43.5%).The genotype distribution of PEAR1 rs2644592 in AS group was 7 cases of GG genotype(12.30%),35 cases of GA genotype(61.40%),and 15 cases of AA genotype(26.30%).3.Chi-square test and t-test showed that the differences in hypercholesterolemia,hypertriglyceridemia,rs12041331 A allele frequency and GA+AA genotype,rs12566888 T allele frequency and GT+TT genotype between AR group and AS group were statistically significant.4.Multivariate Logistic regression analysis showed that PEAR1 rs12566888 GT+TT genotype,hypercholesterolemia and triglyceride may be the risk factors for AR.ConclusionPEAR1SNPrs12566888,Rs12041331 hypercholesterolemia and hypertriglyceridemia may be the risk factors for AR.
