ObjectiveTo observe the short-term prognosis(survival)and treatment status of patients with acute heart failure in the emergency unit of the Second Affiliated Hospital of Anhui Medical University.The difference between the survival group and the death group was compared to study the risk factors affecting the short-term prognosis of patients with acute heart failure,and statistically analyze the prognostic value and significance of each index for acute heart failureMethodsSelect January-December,2019,2017,anhui medical university second affiliated hospital emergency room diagnosis and treatment of 215 cases of patients with acute heart failure,the outpatient service,the hospital electronic medical records system,follow-up methods such as records of patients general situation,laboratory examination,medical history,etc.,according to the situation of patients die within seven days,survival of patients can be divided into 187 people and killed 28people.Statistical analysis was performed by SPSS and Med Calc software.ResultsSingle factor results showed that age,sex,SpO2,C-reactive protein(CRP),neutrophils number,neutrophils ratio,Cr and BUN were the risk factors for acute heart failure,with statistical significance.0.05.Multivariate analysis showed that BUN(OR=1.149,95%CI:1.082~1.220,P<0.001),SpO2(OR=0.939,95%CI:0.938~0.972,P<0.001),age(OR=1.062,95%CI:1.010~1.116,P<0.05),CPR(OR=1.008,95%CI:1.001~1.015,P<0.05),neutrophil count(OR=1.100,95%CI:1.020~1.187,P<0.05)and gender(OR=0.251,95%CI:0.082~0.768,P<0.05)was an independent risk factor for death within 7 days of acute heart failure.The neutrophil ratio(OR=1.009,95%CI:0.964~1.055,P<0.05)and Cr(OR=0.999,95%CI:0.996~1.002,P<0.05)was not a prognostic independent risk factor.Among them,BUN and SpO2 were the best predictors.After grouping according to the optimal cut-off points,Kaplan-Meier survival curve analysis showed that BUN>11.35 mmol/L and SpO2<87%of AHF patients had a significantly higher risk of death within 7 days than the other groups(P<0.001)Conclusion1.Age,sex,CRP,neutrophil count,SpO2 and urea nitrogen were independent risk factors for acute heart failure,among which BUN and SpO2 accounted for the largest weight and had the strongest predictive ability2.BUN and SpO2 can well predict the short-term poor prognosis of patients with AHF,and the prediction effect of the combined prediction of BUN and SpO2 is significantly better than the independent prediction of the two,which can provide an objective basis for clinical evaluation of the prognosis of patients with acute heart failure.
