Objective To investigate the correlation between TyG index and the degree of coronary artery disease in patients with coronary heart disease and adverse cardiovascular events in patients.Methods A total of 466 suspected CHD patients admitted to the Department of Cardiovascular Medicine of our hospital from November 2019 to December 2020 were selected.Basic information,clinical data including laboratory and imaging examination data of all enrolled patients were collected.All the patients were divided into CHD group(n = 242)and NCHD group(n = 224)according to the lesions observed by radiography.In addition,according to the median level of coronary Gensini score(Gensini score was 50 points),242 patients in the CHD group were divided into 122 patients with low Gensini score(Gensini score < 50 points)and 120 patients with high Gensini score(Gensini score ≥50 points).All data of the two groups were statistically compared.Spearman correlation analysis was used to evaluate the relationship between TyG index and coronary Gensini score of patients with CHD,and Logistic regression analysis and ROC curve were used to evaluate the predictive value of TyG index on the degree of coronary artery stenosis in patients with CHD.According to the median TyG index of 8.86,a total of 209 coronary heart disease patients receiving PCI in the CHD group were divided into two groups again,108 patients in the high TyG index group(TyG ≥8.86)and 101 patients in the low TyG index group(TyG < 8.86),to compare whether there was any difference in the incidence of adverse cardiovascular events in the two groups after PCI.Results There was no statistical significance in age,sex ratio,BMI,SCR,BUN,ALT,AST,UA and other data between CHD group and control group.The indexes of diabetes,hypertension,smoking,Hb A1 c,LDL-C,TC,TG and TyG in control group were lower than those in CHD group(P < 0.05),and HDL-C was higher than that in CHD group(P < 0.05).The indexes of diabetes,hypertension,resting heart rate,CTNL,LDL-C,TC,TG,FPG,Hb A1 c,LAD and LVEDD in severe stenosis group were higher than those in mild stenosis group(P < 0.05).HDL-C and LVEF were lower than those in mild stenosis group,and the difference was statistically significant(P < 0.05).There was no significant difference in age,smoking,BMI,SCR,BUN,ALT,AST,UA and other indicators.Logistic regression analysis in this study showed that HDL-C,diabetes and TyG index were independent risk factors for severe coronary artery stenosis.Further ROC analysis showed that compared with triglyceride(AUC 0.758,P=0.035)and fasting blood glucose(FPG)(AUC 0.775,P=0.012),TyG index had the largest area under the ROC curve(AUC 0.831,95%CL 0.762-0.899,P = 0.002).In the comparison of Major adverse cardiovascular events(MACES)in the hospital after surgery,we found that: The proportion of MACES in the high TyG index group was higher than that in the low TyG index group,and the major cardiovascular adverse events were recurrent angina pectoris,acute heart failure and ventricular fibrillation(P < 0.05).There was no statistical significance in cardiac death between the two groups(P > 0.05).Conclusions TyG index was associated with the degree of coronary artery disease in patients with Coronary artery atherosclerotic heart disease and adverse cardiovascular events during hospital stay after PCI.
