Objective To investigate the changes of serum Orexin A(Ox A)concentration on the third day(D3)and the seventh day(D7)after the onset of acute cerebral infarction(ACI)and its relationship with NIHSS score and stroke risk factors.Method 52 ACI patients who were admitted into the Department of Neurology of Chaohu Hospital of Anhui Medical University from November 2019 to September2020 were randomly selected as the experimental group,and 30 physical examiners with the same age group who had no brain imaging evidence and special medical history were selected as the control group.The basic information of all research objects was collected,and venous blood at 11 o’clock before lunch on the day of examination in the control group,fasting venous blood on the morning of the first day after admission,venous blood at 11 o’clock before lunch on the third and seventh days of onset in the experimental group were extracted.The NIHSS scores of admission,the third day and the seventh day of onset in the experimental group were recorded,and the NIHSS scores of the patients on the 30 th day after onset were followed up.The serum Ox A concentrations of D3 and D7 in ACI group and those in the control group at 11 pm(before meals)were compared,and the changes of serum Ox A concentrations of D3 and D7 in ACI patients and their relationship with NIHSS scores and stroke risk factors at each time point were explored.Results 1.There was no significant difference in age,sex,body mass index(BMI)and diabetes between ACI group and control group(P >0.05),but there was significant difference in hypertension history between the two groups(P <0.05).2.There was no significant difference in serum Ox A concentration betweenACI group and control group(P < 0.05).Serum Ox A concentration in ACI group on D7 was higher than that in control group(P < 0.05).In ACI group,serum Ox A concentration on D7 was higher than that on D3,and the difference was statistically significant(P <0.05).3.There was a negative correlation between serum Ox A concentration on D3 and NIHSS score on D3 in ACI group(r=-0.089,P=0.039).There was no correlation between serum Ox A concentration on D7 and NIHSS score in ACI group(P > 0.05).In ACI group,there was no correlation between the difference of serum Ox A concentration between D7 and D3 and the changes of NIHSS score before and after admission(D3-D7-D30)(P >0.05).4.There was no correlation between serum Ox A concentration and hypertension,diabetes history,homocysteine,total cholesterol,triglyceride,high density lipoprotein and low density lipoprotein in ACI group at D3 and D7 after onset(P > 0.05).5.There was a positive correlation between serum Ox A concentration and BMI in ACI group D3 after onset(P < 0.001),and the correlation coefficient was 0.708.The serum Ox A concentration in the control group was positively correlated with BMI(P=0.006),and the correlation coefficient was 0.494.However,there was no correlation between serum Ox A concentration and BMI in ACI group D7 after onset(P > 0.05).Conclusion 1.The serum Ox A concentration in ACI patients is related to the onset time,and the serum Ox A concentration in D7 is higher than that in D3.Ox A may participate in the protection mechanism of neurological function in the early stage of ACI,which is related to the early neurological function of patients,but has nothing to do with the long-term recovery of neurological function of patients.2.There was no correlation between serum Ox A concentration and hypertension,diabetes history,homocysteine,total cholesterol,triglyceride,high density lipoprotein and low density lipoprotein in ACI patients.But the serum Ox A concentration is related to BMI value,When BMI value is in different ranges,the correlation between BMI value and serum Ox A may be different.When BMI value ranges from 18.5 to 28,serum Ox A concentration is positively correlated with BMI value.However,in the early stage after ACI,this correlation still exists.In the later stage of ACI,this correlation may be changed by drugs or other factors.
