| Objective To compare the efficacy and safety of transurethral 1470nm diode laser vaporization of the prostate(1470nm Di LVP)and transurethral greenlight photoselective vaporization of the prostate(PVP)in the treatment of benign prostatic hyperplasia.Methods Data of 204 patients with benign prostatic hyperplasia admitted to the Sixth Medical Center of PLA General Hospital,the former Navy General Hospital from January 2017 to September 2019 were used to investigate.Among them,103 patients were treated by transurethral 1470nm semiconductor laser vaporization of the prostate,and the other 101 patients were treated by transurethral green laser vaporization of the prostate.Blood loss during operation,operation time,postoperative bladder irrigation time,postoperative indwelling catheter time,postoperative hospital stay and complications were compared between the two groups.The International Prostate Symptom Score(IPSS),Quality of Life Score(Qo L)and Maximum Urinary Flow Rate(Qmax)were statistically compared before operation,3 months after operation and 1 year after operation.And the incidence of postoperative complications such as transient urinary incontinence,postoperative gross hematuria,and postoperative reverse ejaculation.Results The results showed that the comparison between group is blood loss during operation(14.7±7.7 vs.16.4±8.8 ml,p>0.05),operation time(89.1±38.9 vs.82.4±39.6min,p>0.05),postoperative bladder irrigation time(20.6±4.0 vs.22.0±6.4 h,p>0.05),postoperative indwelling catheter time(71.2±17.2 vs.73.7±21.5 h,p>0.05),postoperative hospital stay(5.0±1.0 vs.5.3±1.2 d,p>0.05)and complications[2.9%(3/103)vs.4.0%(4/101),p>0.05].The IPSS of the Di LVP group decreased from24.0±3.9 points before surgery to 8.0±3.7 points at 3 months after surgery and6.0±1.9 points at 1 year after surgery;Qo L in the Di LVP group decreased from4.9±1.1 points before surgery to 3 months after surgery 1.3±0.6 points,1.3±0.6points 1 year after surgery;Qmaxof Di LVP group increased from 4.9±2.2ml/s before surgery to 15.4±4.0ml/s at 3 months after surgery,and 16.5±2.4ml/s at 1year after surgery;The IPSS of the PVP group decreased from 23.9±3.2 points before surgery to 7.5±3.3 points at 3 months after surgery and 6.1±1.8 points at1 year after surgery;Qo L in the PVP group decreased from 4.7±0.9 points before surgery to 3 after surgery Month 1.4±0.5 points,1 year after surgery1.4±0.6 points;PVP group Qmaxincreased from 4.6±1.9ml/s before surgery to15.4±3.8ml/s at 3 months after surgery,and 16.4±2.2ml/s at 1 year after surgery.Preoperative and postoperative 3 months,1 year after operation IPSS,Qo L and Qmaxwere compared between groups have no statistical difference(p>0.05).Group comparison before and after the operation there are significant differences(p<0.01).There was no significant difference in the incidence of postoperative complications such as transient urinary incontinence,gross hematuria,and reverse ejaculation after surgery(p>0.05).Conclusions Transurethral 1470nm diode laser vaporization of the prostate and transurethral greenlight photoselective vaporization of the prostate are effective and safe for the treatment of benign prostatic hyperplasia,and they have considerable safety and therapeutic effects. |
