Perioperative Influencing Factors Of Intestinal Function Reconstruction After Congenital Intestinal Atresia

Objective Congenital intestinal atresia is a kind of congenital malformation which seriously affects the life of children.Operation is the only way to treat it.Different conditions of children,location of intestinal atresia and age of operation may lead to different postoperative intestinal function recovery.This paper aims to analyze the influencing factors of postoperative intestinal function recovery of congenital intestinal atresia,assist clinical diagnosis and treatment,and improve the prognosis.Methods Congenital intestinal atresia refers to the intestinal atresia or interruption of children.The 85 children with congenital intestinal atresia were diagnosed in Xinjiang Uygur Autonomous Region People’s Hospital from January 1,2009 to Janunary 31,2020.The investigation was carried out with a unified registration form.According to the inclusion criteria:(1)the clinical data were complete;(2)the diagnosis was confirmed as intestinal tract according to clinical manifestations,related examinations(gastrointestinal radiography,abdominal X-ray film,laparotomy,etc.)and postoperative diagnosis The results showed that the patients were locked up;(3)they met the indications of operation and accepted surgical treatment,and the exclusion criteria were as follows:(1)family members gave up treatment;(2)the children died and 66 cases finally met the conditions.According to the time of feeding the whole intestine,the two groups were divided into early recovery group(< 10 days)and late recovery group(>10 days).According to the first feeding time after operation,the group was divided into early weaning group(< 7 days)and late weaning group(> 7 days).According to the hospitalization time,the two groups were divided into short-term residential group(< 20days)and long-term hospitalization group(> 20 days).The sex,out of hospital group and the long-term hospitalization group(> 20 days)were recorded respectively Birth weight,gestational age,admission day,operation day,mother’s age,admission weight,weight at operation,operation mode,locking position,preoperative hemoglobin,preoperative total protein,preoperative albumin,preoperative blood sodium,preoperative blood potassium,postoperative hemoglobin,total protein after operation,postoperative albumin,postoperative blood sodium,postoperative blood potassium,pneumonia,peritonitis and sepsis And the first time of defecation after operation.The above factors were tested by single factor 2,and the difference was analyzed in the time of feeding the whole intestine,the first feeding time after operation and the hospitalization time(P < 0.05).Then,the factors of difference were included in the logistic regression analysis of the two categories to analyze the factors that affected the recovery of the children after the operation.Results 1.Among 66 cases,23 cases(34.8%)were in early recovery group and 43cases(65.2%)were in late recovery group.The related factors were analyzed by univariate 2 test.The age of operation,operation mode,position of atresia,postoperative hemoglobin and postoperative blood sodium P values of the two groups were 0.01,<0.01,0.01,0.01 and 0.01,respectively,and the differences were statistically significant.Logistic regression analysis showed that there were significant differences in age of operation and operation method(P = 0.03,P<0.01).2.Among the 66 cases,29 cases(43.9%)were in the early weaning group and37 cases(56.1%)were in the late weaning group.The related factors were analyzed by univariate 2 test.The P values of admission age,atresia position,operation method,postoperative sodium,gestational age,sepsis and the first defecation time were 0.04,<0.01,<0.01,0.04,<0.01 and <0.01,respectively Righteousness.Logistic regression analysis was used for multivariate analysis.The results showed that there were significant differences in age of admission and operation methods(P <0.01).3.Among the 66 cases,34 cases(51.5%)were in the short-term hospitalization group and 32 cases(48.5%)were in the long-term hospitalization group.Univariate analysis showed that the age of admission day,gestational age and the first defecation time after operation of the two groups were 0.03,0.01 and 0.02,respectively,and the differences were statistically significant.The above three factors were included in binary logistic regression analysis for multivariate analysis,the results showed that there was no significant difference(P > 0.05).Conclusions 1.Compared with the late recovery group,the age of operation within 7days after birth and intestinal anastomosis were the influencing factors for the recovery of intestinal function in the early recovery group.2.Compared with the late weaning group,the age of admission,within 3days after birth and intestinal anastomosis were the influencing factors for the recovery of intestinal function.In conclusion,standardizing maternal prenatal examination,early diagnosis and early operation are conducive to the recovery of postoperative intestinal function in children with intestinal atresia.
