| Objective:To explore the influencing factors of cognitive function and the characteristics of cognitive domain in elderly patients with chronic obstructive pulmonary disease(COPD)and mild cognitive impairment(MCI).Methods:Collect patients with chronic obstructive pulmonary disease(COPD)treated in our hospital from November 2019 to December 2020,COPD patients were divided into two subgroups according to the diagnostic criteria of mild cognitive impairment(MCI):there were 33 cases in the COPD combined with MCI group in group A,34 cases in the simple COPD group in group B,30 cases of MCI patients with similar education levels as those with COPD combined with MCI were recruited at the same time,and set as group C.The clinical data of the two groups A and B were compared,and the factors affecting the cognitive function of COPD patients were analyzed by Logistic regression.At the same time,compare the cognitive domains of group A and group C to explore the characteristics of cognitive domains of COPD and MCI patients.Results:1.general data comparison:the MMSE and MoCA of the A group were lower than those of the B group,but there was no significant difference in age,sex and years of education.There was no significant difference in age,years of education,MMSE and MoCA between the A group and the C group,but there was a statistical difference in sex.There were more male patients in the group than in the group.2.the characteristics of cognitive domain:There was no significant difference in the overall cognitive function between group A and group B[MMSE and MoCA in 33 cases(25.52±2.35,19.52±2.61)vs.30 cases(26.37±2.06,20.77±3.29,P=0.133,0.098)].AVLT(delayed recall)[(2.48±1.81)vs.(4.60±3.64),P=0.006], AVLT(immediate recall)[(12.45±3.10)比(16.67±3.85),P<0.001],CDT [(15.55±6.11)vs.(25.40±7.09),P<0.001],CFT[(23.12±5.99)vs.(29.12±2.41),P<0.001],the time in TMT-A[(79.80±31.09)vs.(59.57±29.38),P=0.010],TMT-B[(22.58±2.03)vs.(23.73±0.69),P=0.004]in group A were lower than those in group C.3.Influencing factors:Logistic regression analysis showed that oxygen partial pressure,FEV1% of projected and serum albumin were the influencing factors of cognitive function in COPD patients.Conclusion:1.The impairment of cognitive function in elderly patients with COPD is mainly manifested in visual space,executive function and memory function,of which the decline in visual space is the most significant.2.The main influencing factors of elderly COPD patients’cognitive function are oxygen partial pressure,FEV1% pred and serum albumin level. |
