Objective:To compare the mid-and long-term clinical effects of ACDR and ACDF in the treatment of single-level cervical spondylosis in China. Methods:Pub Med,Embance,CNKI,Wanfang and VIP databases were searches for randomized controlled trials addressing ACDR versus ACDF for cervical spondylosis in China punlished from January 2010 to December 2020.Two reviewer independenly read and screened literature, extracted the date, and evaluated the quality.Meta-analysis was performed using Rev Man 5.4 software. Results:Nine trials were included, involved 856 participants. The participants were follow-up for 4-10 years. The results of this meta-analysis indicated that :compared with ACDF,there is no significant difference in the VAS of neck pain and the score of the JOA in the mid-and long-term follow-up after the operation. In terms of dysfunction score,ACDR is better than ACDF.(2): ACDR can effectively preserve the ROM of the surgical segment and the overall range of motion of the cervical spine than ACDF,so that the overall structure of the cervical spine is closer to the normal physiological state.(3):ACDR also has related complications, but the incidence of secondary operations after cervical disc replacement is lower. Conclusion:ACDR in the treatment of cervical spondylosis in my country has certain advantages over ACDF,but this conclusion requires a larger sample and more center randomized controlled trials to further demonstrate.
