Objective:To analyze the characteristics of solitary pulmonary nodule(SPN)by using multi-parameter imaging of dual-layer spectral detector CT(SDCT).Methods:A total of 56 SPN patients who received two-phase enhanced SDCT scan and were pathologically confirmed from February 2020 to January 2021 were collected and separated into group one-benign and group two-malignant,the former including21 cases and the latter including 35 cases.Measure and calculate the average iodine concentration(IC),the mean effective atomic number(Eff-Z),Normalized iodine concentration(NIC)of the lung,Normalized iodine concentration in the proximal region(NICpro),Normalized iodine concentration in the distal region(NICdis)and the differences in NIC(d NIC)between the proximal region and the distal region.T test was used to compare the differences among the parameters,and the ROC curves and thresholds of each parameter were obtained for the differential diagnosis of SPN.Results:The difference of d NIC in the venous phase of SPN between the two groups was statistically significant(P<0.05).The NICpro in malignant group was greater than NICdis.When d NIC ≥ 0.00mg/ml,the sensitivity and specificity were 74.3% and52.4%,respectively.In addition,there were statistically significant differences in IC,NIC and Eff-Z of SPN between the two groups in arterial venous phase(P<0.05),and the diagnostic efficiency in venous stage was higher than that in arterial stage.When the NIC≥0.22mg/ml in venous phase,the sensitivity and specificity were 91.4% and61.9%,respectively.Conclusion:SDCT using the spatial distribution difference of iodine concentration is helpful to the differential diagnosis of benign and malignant SPN,and provides a new qualitative method for the differential diagnosis of SPN;SDCT multi-parameter imaging is expected to provide more clinical application value for the differential diagnosis and evaluation of SPN.
