Objective:Immune inflammation plays an important role in chronic obstructive pulmonary disease(COPD)and acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Inducible bronchus-associated lymphoid tissue(i BALT)is closely related to the abnormal immune response of COPD.Current studies indicated that i BALT has a beneficial effect on patients with acute inflammation and an bad effect on patients with chronic inflammation,the role of acute exacerbation in patients with chronic inflammation still not clear.Therefore,to analyze the level and significance of bronchus-associated lymphoid tissue(i BALT)related to CXCL13 in bronchoalveolar lavage fluid of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD),it is helpful for providing the basis for precision medicine of patients with AECOPD.Methods:14 patients with AECOPD caused by bacterial infection were selected as AECOPD group,and 24 patients with acute bacterial pneumonia in the same period were selected as infection group.The bronchoalveolar lavage fluid of the two groups was collected by bronchoscope,and the levels of CXCL13,IL-8,IL-6 and TNF-α in the lavage fluid were detected by ELISA.The levels of CXCL13,IL-8,IL-6 and TNF-α were compared between the two groups,and the correlation between CXCL13 and IL-8,IL-6,TNF-α were analyzed.Results:CXCL13(P=0.016),IL-8(P=0.082),IL-6(P=0.008)and TNF-α(P=0.201)in AECOPD group were higher than infection group.In AECOPD group.CXCL13 were negative correlated with IL-8(r=-0.925,P<0.001)and TNF-α(r=-0.886,P<0.001).In infection group,CXCL13 were negative correlated with IL-8(r=-0.789,P<0.001),and TNF-α(r=-0.480,P=0.018).There were no correlation between CXCL13 and IL-6 in the two groups(P>0.05).Conclusion:1.CXCL13 was significantly increased in patients with AECOPD compared with patients with acute bacterial pneumonia in the alveolar lavage fluid of.2.CXCL13 is negatively correlated with the inflammatory factors of IL-8 and TNF-αin patients with AECOPD infected by bacteria.
