| Objective To investigate the effect and mechanism of dexmetomidine on sleep quality in patients with chronic insomnia. Methods Sixty patients with chronic insomnia in the department of sleep disorders in our hospital from August 2019 to June 2020,24 males and 36 females,ASAⅠorⅡ,aged from 18 to 60 years old, were randomly divided into two groups: Dexmetomidine treatment group(DEX group,n=30)and control group(group C,n=30).After entering the room, routine ECG monitoring was performed and dexmetomidine was given to induce sleep under polysomnography(PSG).The changes of Pittsburgh sleep quality index(PSQI),cognitive function score(MOCA)and hemodynamics were observed before and after treatment in the two groups. At the same time, the concentrations of serum S100 B protein and Hepcidin were detected.Pearson test was used to analyze the correlation between S100 B protein and Hepcidin concentration. Results Compared with group C,the PSQI score of DEX group decreased after treatment, but the MOCA score had no significant change. The results of PSG showed that the total sleep time, sleep efficiency, sleep latency,N2 sleep and N1 sleep in DEX group were longer than those in group C after treatment. There was no significant difference in hemodynamic changes between the two groups. In DEX group, the concentrations of serum Hepcidin and S100 B protein decreased after 3 days of treatment, and there was a certain correlation between them. However,on the 7th day after treatment, the concentration of serum S100 B protein increased again. Conclusion DEX can correct the disorder of brain iron metabolism in patients with chronic insomnia by inhibiting iron death and improve the sleep quality of patients with chronic insomnia. |
