Objective:To investigate the correlation between common and common syndromes and neutrophil-lymphocyte ratio in patients with acute ischemic stroke,and to observe whether there is a characteristic correlation between them.So as to provide a possible objective basis for the syndrome differentiation of acute ischemic stroke patients.Methods: 1.In this study,220 patients with acute ischemic stroke in the Department of Neurology,Jiangxi Provincial People’s Hospital from December 2019 to December 2020 were selected.The patients ranged in age from 50 to 80.The ratio of mesenchymal granulocytes to lymphocytes was calculated.According to the criteria for the diagnosis of stroke in the7 th edition of Internal Medicine of Traditional Chinese Medicine,four syndrome types were included: phlegm-fire stasis group,phlegm-heat fu-solid group,wind-yang up-disturbance group,and yin-deficiency wind-motion group.According to Professor Zhu Wenfeng’s "Syndrome Differentiation of Syndrome Elements",the syndrome elements of five common diseases of acute ischemic stroke patients including phlegm,heat,blood stasis,Yin deficiency and moving wind were integrated.The correlation between common and common syndromes and the ratio of neutrophils to lymphocytes in patients with acute ischemic stroke was analyzed statistically.2.SPSS 23 statistical software was used for statistical analysis,and the statistical results were as follows: P>0.05 was no statistical difference;P<0.05,P<0.01 was considered statistically significant.Results:1.This study shows that there is a statistical difference in the distribution of TCM syndromes of 220 patients with acute ischemic stroke in gender,P<0.05.The overall distribution was as follows: 75 patients with phlegm and fire stasis syndrome were included,49 male patients and 26 female patients,respectively.Forty-nine cases of phlegm-heat fu-organs were included,26 male and 23 female patients,respectively.Sixty-one patients with Wind-Yang up-disturbance syndrome were included in the study,including 33 males and 28 females.Thirty-five patients with Yin deficiency and wind motion syndrome were included,including 12 male patients and 23 female patients,respectively.In terms of age distribution,there was no statistical difference between the distribution of TCM syndromes and age(P>0.05).Yin deficiency and moving wind syndrome(66.26±9.13)> phlegm fire stasis closed syndrome(65.55±7.69)> phlegm heat and fu-organs positive syndrome(65.02±8.73)> wind Yang disturbance syndrome(64.66±9.13).The ratio of neutrophils to lymphocytes is different in different TCM syndromes.NLR from small to large: Yin deficiency wind movement syndrome,wind Yang disturbance syndrome,phlegm heat and fu-organ evidence,phlegm fire stasis syndrome.The syndrome of phlegm and fire stasis was significantly higher than that of phlegm and heat in fu-organs,the syndrome of wind-yang disturbance,and the syndrome of Yin deficiency and wind movement(P<0.05).The positive evidence of phlegm-heat and fu-organs was significantly higher than that of wind-yang up-disturbance syndrome and Yin deficiency wind-motion syndrome(P<0.05).There were significant differences in NLR level among all groups(P<0.05).There was no statistical difference in > in wind-yang up-disturbance group(P>0.05).There were statistical differences between the other two groups(P<0.05).2.220 cases of acute ischemic stroke patients in terms of TCM disease syndromes,patients with different syndromes have statistical differences in gender.The sputum group included 137 patients,including87 male patients and 50 female patients(P<0.05).141 patients were enrolled in stasis group,including 89 males and 52 females(P<0.05).114 cases were enrolled in Yin deficiency group,including 50 male patients and 64 female patients(P<0.05).In the dynamic wind group,133 patients were enrolled,including 85 male patients and 48 female patients(P<0.05).In terms of the age distribution of syndromes and interstitials,there was a statistical difference between the syndromes and interstitials.The average age of the phlegm and stasis group was 65.20±8.17,the average age of the stasis group was 61.53±7.90,the average age of the phlegm and stasis group was 58.03±4.20,and the average age of the phlegm and stasis group was 70.03 ± 6.71,P<0.05.In the phlegm-heat-cum-clip group,the mean age of the phlegm-heat-cum-clip group was 66.79±6.93,the mean age of the heat group was 58.68±5.31,the mean age of the phlegm-heat-cum-clip group was 62.24±8.27,and the mean age of the phlegm-heat-cum-clip group was 69.51±7.76,P<0.05.In the Yin deficiency and moving wind combined clip group,the mean age of the Yin deficiency group was 65.51±7.48,the mean age of the moving wind group was 64.68±8.91,the mean age of the non-Yin deficiency and moving wind group was 61.84±7.10,and the mean age of the Yin deficiency and moving wind group was 67.80±8.31,P<0.05.In terms of the correlation between NLR and syndrome elements,the syndrome element scores of the phlegm,heat and stasis groups were positively correlated with NLR(P<0.01).There was no significant positive correlation between syndrome element score and NLR in Yin deficiency group and dynamic wind group(P>0.05).The correlation between syndrome element score ≥20 points and NLR level was further investigated.When the score of phlegm group ≥20 points,heat group ≥20 points,and stasis group ≥20 points,the NLR level was high(P<0.01),and there was a positive linear correlation between the two.Conclusion: 1.The distribution of TCM syndromes in patients with acute ischemic stroke is related to gender.Male patients have higher probabilities of phlegm-fire stasis syndrome,wind-yang up-disturbance syndrome,phlegm-heat fu-fu syndrome than female patients;The probability of Yin deficiency and wind motion syndrome in female patients is higher than that in male.The common TCM syndromes of acute ischemic stroke patients are not related to age.2.The ratio of neutrophils to lymphocytes in acute ischemic stroke patients with phlegm-fire stasis was significantly higher than that of phlegm-heat fu-fu syndrome,wind-yang up-disturbance syndrome and Yin deficiency wind-motion syndrome.The ratio of neutrophils to lymphocytes in phlegm-heat fu-organs was significantly higher than that in wind-yang up-disturbance syndrome and Yin deficiency wind-motion syndrome.3.In terms of disease syndromes,the TCM syndromes of acute ischemic stroke patients are related to gender to some extent.The syndrome elements of "phlegm","blood stasis" and "moving wind" are commonly seen in males,while the syndrome elements of "Yin deficiency" are commonly seen in females.In terms of syndromes and syndromes,TCM syndromes and syndromes are related to age,and patients in the group of syndromes and syndromes are older.4.There was a positive linear correlation between the scores of phlegm,heat and stasis syndromes and the ratio of neutrophils to lymphocytes in patients with acute ischemic stroke.The higher the scores of syndromes,the greater the NLR.Therefore,the study of syndrome element score and NLR level in patients with acute ischemic stroke can further guide TCM clinical syndrome differentiation,especially to provide possible objective basis for patients with phlegm,heat and blood stasis syndrome.
