| Objective:Selecting thermal moxibustion advantage diseases of primary dysmenorrhea as the research object,to close yuan of acupuncture point to stimulate the parts,heat-sensitive moxibustion therapy as a means of intervention,application of resting state fmri technology,using its own before and after the comparison,analysis of low frequency amplitude and regional homogeneity,discuss the spontaneous neuronal activity and function of brain regions local consistency of coordinating role with immediate effect.Methods: A total of 18 PD patients in the outpatient department of the Acupuncture and Moxibustion Department of Jiangxi University of Traditional Chinese Medicine from February 2019 to January 2020 were selected,and the heat-sensitive moxibustion sensation can be detected at Guanyuan point.Baseline data such as age,weight,height,etc.were calculated with SPSS25.0.Use the 3.0T functional magnetic resonance instrument from GE Company of the Affiliated Hospital of Jiangxi University of Traditional Chinese Medicine,and the equipment model is Sign HDx 3.0T.T1 structure image,pre-excitation functional image,and excitation functional image scan were performed on all patients.The scanning time is 6 minutes.The data preprocessing application is based on MATLAB2013b’s RESTplus_V1.2 software package,which sequentially performs DICOM to NIFTI data format conversion,removal of the first 5 time points,time correction,head movement correction,image registration,anatomical image segmentation,spatial standardization,spatial smoothing,Steps to remove linear drift and filtering.The statistical application SPM12 is used for the secondorder analysis,using its own before and after control design.Calculate the mALFF value to detect the spontaneous activity of neurons;calculate the ReHo value to detect the consistency of brain functional activity in the region.Use xjview95 to obtain the corresponding image,the volume threshold of each cluster block is greater than or equal to 85 voxels,and the single voxel threshold p <0.05.Image visualization is based on Brain Net to obtain the final result image.Results:1.Baseline data: All subjects were(23.78±2.34)years old,weight(51.39±5.17)KG,height(160.72±4.53)CM;CMSS average pain score(12.13±1.8);VAS score(5.62±1.58)CM;average course of disease(6.10±4.63)years;heat-sensitive moxibustion is mainly manifested as diathermy 78%,heat expansion 89%,and heat transfer 44%;all are right-handed.2.Brain regions with significant differences in mALFF values(p <0.05,uncorrected,voxels ≥85):(1)The brain areas enhanced by mALFF are concentrated in 5 clumps: clump No.1 is mainly distributed with Hippocampus_L,Precuneus_L,Para Hippocampal_L,and Cuneus_L;clump No.2 is mainly distributed with Frontal_Mid_R,Frontal_Mid_L,and Frontal_Inf_Tri_L;clump No.3 is mainly distributed with Frontal_Mid_R,Frontal_Mid_L and Frontal_Inf_Tri_L.Hippocampus_R,Amygdala_R;No.4 clump is mainly distributed with Precuneus_R;No.5 clump is mainly distributed with Parietal_Sup_R and Angular_R.Among them,the largest distribution range of mALFF is clump 2,with 2718 voxels;the smallest distribution range of mALFF is clump 4,with 97 voxels.For different clumps,the largest mALFF value is Frontal_Mid_R,followed by Frontal_Mid_L,which are all distributed in the 2nd clump.In different clumps,the largest proportion of brain voxels is Precuneus_R,reaching 88%.(2)The brain area with reduced mALFF is concentrated in two clumps: No.1 clump is mainly distributed in Temporal_Sup_L;No.2 clump is mainly distributed in Cuneus_R,Occipital_Sup_R,Occipital_Mid_R.Among them,the largest distribution range of mALFF is clump 2,with 290 voxels;the smallest distribution range of mALFF is clump 1,with 137 voxels.For different clumps,the largest mALFF value is Cuneus_R,followed by Occipital_Sup_R,which are all distributed in the 2nd clump.For different clumps,the largest proportion of brain voxels is Temporal_Sup_L,up to 35%.3.Brain regions with significant differences in ReHo values(p <0.05,uncorrected,voxels ≥85):(1)The brain areas enhanced by ReHo are concentrated in 8 clumps: clump 1 is mainly distributed in Temporal_Sup_R,Temporal_Mid_R,Temporal_Pole_Sup_R;clump 2 is mainly distributed in Temporal_Pole_Sup_L,Temporal_Mid_L,Angular_L,Frontal_Inf_campus_Para_L,and Haracampus;The distribution of clump 4 mainly includes Caudate_R and Insula_R;the distribution of clump 5 mainly includes Angular_R,Temporal_Mid_R,and Supra Marginal_R;the distribution of clump 6 mainly includes Frontal_Mid_R,Frontal_Mid_L,Frontal_Sup_R;the distribution of clump 7 mainly includes Oc Precipital_Mid_Mid_L,Postcentral_R,Rolandic_Oper_R.Among them,the largest ReHo distribution range is clump 6,with 1413 voxels;the smallest ReHo distribution range is clump 7,with 126 voxels.For different clumps,the largest ReHo value is Frontal_Mid_R with a voxel volume of 237,followed by Frontal_Mid_L with a voxel volume of 220.Occipital_Mid_L accounts for the largest proportion of brain voxels in different clumps,reaching 95%.(2)The brain area reduced by ReHo is concentrated in two clumps: clump 1 is mainly distributed with Precentral_L and Postcentral_L;clump 2 is mainly distributed with Cingulum_Mid_L,Cingulum_Mid_R,and Supp_Motor_Area_L.The weakest is the left central anterior gyrus,followed by the left medial and lateral cingulate gyrus.Among them,the largest ReHo distribution range is clump 1,with 345 voxels;the smallest ReHo distribution range is clump 2,with 167 voxels.For different clumps,the largest ReHo value is Precentral_L with a voxel volume of 146,followed by Postcentral_L with a voxel volume of 99.For different clumps,the largest proportion of brain voxels is Precentral_L,reaching 41%.Conclusion: 1.Moxibustion at Guanyuan point in the heat-sensitive state can significantly activate the prefrontal system(PFC),which may be closely related to the key emotional analgesia.2.Moxibustion at Guanyuan acupoint in the heat-sensitive state can enhance the spontaneous neuron activity of the middle frontal gyrus and the precuneus;reduce the spontaneous neuron activity of the cuneiform,superior occipital gyrus and the middle occipital gyrus.The coordination of local functional activities of the brain regions represented by the middle frontal gyrus and the superior temporal gyrus was enhanced;the coordination of local functional activities of the brain regions represented by the central anterior gyrus and the central posterior gyrus was weakened.3.The immediate effect of moxibustion at Guanyuan acupoint in the heat-sensitive state is significant,and the brain area is widely activated,indicating that there is a networked central control of the acupoints in the heat-sensitive state of moxibustion. |
