| Objective: To investigate the contraceptive status and needs of the subjects in one year after delivery through investigation and analysis,so as to provide scientific theoretical basis for further strengthening the publicity and education of knowledge related to postpartum contraception and birth control,promoting the contraceptive mode suitable for the majority of postpartum women and improving the reproductive health level of postpartum women.Methods: A total of 779 women who delivered in the third obstetrics department of Northwest Women and Children’s Hospital on November 1,2019 and November 30,2019 were selected as the research subjects.A self-made questionnaire was used to follow up within 1 year after delivery,and the general situation,postpartum contraceptive status and contraceptive needs of the research subjects were collected.Epi Data 3.1 was used to establish the database,and SPSS 25.0 was used for statistical analysis.Results:1.The average time of breastfeeding,menstrual recovery and sexual recovery of the subjects in this study was 8.70 months,5.16 months and 4.94 months,respectively.Multivariate logistic regression analysis showed that whether previous contraception,postpartum breastfeeding time,postpartum menstrual recovery time and postpartum sexual recovery time were related.2.77.41% of the parturients took contraceptive measures,and the average time to start using contraceptive measures was 4.90 months.Among them,condoms(69.32%),external ejaculation(5.01%)and safe period(1.67%)were the most commonly used contraceptive methods after delivery.Multivariate logistic regression analysis showed that the choice of contraceptive method was related to whether or not the previous contraceptive method and the most common contraceptive method used in the past.3.The overall correct rate of the subjects’ knowledge of postpartum contraception was11.4%,and whether to resume sex life after delivery is related to the knowledge of contraception after delivery.71.50% of the women were satisfied with the counselling work on postpartum contraception,and 84.47% of the women thought it was due to insufficient publicity and attention;86.65% of parturients believe that condoms are the best method of contraception after delivery.4.Within 1 year after delivery,3.34%(26/779)of the parturients had an unintended pregnancy,and 61.54% chose artificial abortion to terminate pregnancy.Multivariate logistic regression analysis showed that long postpartum breastfeeding time was associated with the occurrence of unplanned postpartum pregnancy.5.In this study,94.22% and 87.80% women considered it necessary to carry out postpartum contraceptive education and were willing to accept counseling and guidance,respectively.The most preferred approaches were public media such as We Chat(47.24%),personalized guidance from medical staff(25.03%),and community health service centers(10.40%).Conclusion:1.Condoms,external sperm extraction,safe period and other contraceptive methods are used by most women in childbirth.There are fewer options of long-acting reversible contraceptive measures after delivery,and most of the women have a low awareness rate of contraceptive knowledge after delivery.Relevant medical personnel should strengthen the education on contraception during pregnancy and childbirth,and urge the implementation of contraceptive measures.2.Pregnant women with early menstrual recovery time are more likely to resume sexual life in early postpartum;Women who breastfed within six months of giving birth were more likely to resume sex within four to six months;Pregnant women with menstrual recovery beyond 3 months postpartum were more likely to resume sexual activity 7 to 12 months postpartum;Pregnant women who did not use contraception in the past,breastfed within six months after delivery,and resumed menstrual time within six months after delivery were less likely to resume sexual life within one year after delivery.3.Parturient women who had previously used high efficiency contraception and physical barrier contraception tended to choose high/medium efficacy contraception after delivery;Pregnant women who had not previously used contraceptives tended to choose less effective contraceptives/non-contraceptives after delivery.4.Women who breastfeed for a long period of time are more likely to have an unplanned pregnancy after delivery.5.Most of the puerpera consider it necessary and are willing to accept the postpartum contraceptive propaganda,education and consultation guidance,which are followed by public media such as We Chat official accounts and personalized guidance from medical personnel. |
