Clinical Observation On The Application Of Constitution Identification And Three Differentiation Diagnosis And Treatment Mode In Chronic Gastritis

Objective :Through the discussion constitution syndrome type distribution rule types of patients with chronic gastritis,and to observe the clinical efficacy of three differentiation diagnosis and treatment mode.Objective to summarize the clinical experience of director Tang Yi in the treatment of chronic gastritis,and to provide new clinical ideas and basis for the application of three differentiation diagnosis and treatment mode in the treatment of chronic gastritis.Research MethodThrough epidemiological investigation,the constitution of patients with chronic gastritis was evaluated by constitution identification scale,syndrome type judgment scale and symptom score scale,and the data were recorded.The oral decoction was the medicine that director Tang Yi used three differentiation diagnosis and treatment mode to treat.The symptom product scale was retested after 8 weeks of treatment.All data were analyzed by spss21.0 statistical software.Research results1.General situation of 151 patients with chronic gastritis: there was no difference in gender distribution.The age distribution is mainly young people,accounting for53%.The distribution of BMI was normal in 55.6%,overweight and obesity in 36.4%.Overweight and obesity were the main factors in men,accounted for 57.8%,while normal weight was the main factor in women,accounted for 67.85%.The BMI of Hp positive patients was higher than that of Hp negative patients.2.Distribution of constitution types: the constitution type of 151 patients with chronic gastritis was Yang deficiency,accounting for 27.8%,followed by damp heat,accounting for 16.6%.The most common constitution was deficiency constitution(48.3%),followed by solid constitution(46.4%)and ideal constitution(5.3%).In male patients,the constitution type of damp heat type accounted for the most,accounting for 26.6%,Yang deficiency type,phlegm dampness type accounted for 18.8%,and blood stasis type accounted for the least,accounting for only 1.6%.The top three constitution types of female patients were Yang deficiency(34.5%),Qi deficiency(14.9%)and qi depression(14.9%),and the proportion of mild constitution was only2.3%.The type of constitution is mainly of virtual constitution,accounting for 48.3%,followed by solid constitution,accounting for 46.4%,and mild constitution,accounting for 5.3%.The constitution type of Hp positive patients was mainly damp heat type,accounting for 27.1%,followed by Yang deficiency type,accounting for18.6%.The constitution types of youth group were mainly Yang deficiency(26.3%)and damp heat(23.8%).The middle-aged group was dominated by Yang deficiency(26.2%)and Qi deficiency(19.6%).3.Distribution of syndrome types: The main syndrome type of chronic gastritis was weakness of spleen and stomach(43%),followed by damp heat of spleen and stomach(21.9%)and disharmony of liver and stomach(21.2%).The damp heat of the spleen and stomach accounted for37.5% in males.In this study,no stomach collateral stasis syndrome was found in males.The weakness of spleen and stomach accounted for 51.7% of the female.Five cases of stomach collateral stasis syndrome were female.Hp positive cases were mainly spleen stomach damp heat syndrome,accounting for44.3%.Hp negative spleen stomach damp heat syndrome accounted for the least,only two cases.Mainly syndrome is spleen stomach weakness of all ages,accounting for54.3%.The second syndrome type was spleen stomach damp heat syndrome in young group and middle-aged group,and disharmony of liver and stomach syndrome in elderly group,accounting for 36%.4.The correlation between constitution and syndrome types of chronic gastritis:Yang and Qi defect constitution are related to spleen and stomach deficit;damp heat and phlegm dampness constitution are related to spleen stomach damp heat;qi depression and yang deficiency constitution are related to disharmony between liver and stomach;It matters in Yin deficiency constitution and deficiency of stomach yin,and in blood stasis constitution and blood stasis of stomach collaterals5.Analysis of the curative effect of chronic gastritis in this study: Overall efficiency was 87.4%,6% were cured,29.8% were markedly effective,51.6% were effective and 12.6% were ineffective.The application of three differentiation diagnosis and treatment mode can improve the symptoms,but belching hiccup has no improvement.The curative effect of damp heat syndrome is better than that of blood stasis syndrome,and spleen stomach damp heat syndrome is better than stomach collateral stasis syndrome.Research Conclusion1.The results of this study showed that the TCM constitution type of patients with chronic gastritis was mainly Yang deficiency,mostly damp heat.Spleen and stomach deficiency syndrome is the main syndrome type,and spleen and stomach damp heat syndrome is the most common.Chronic gastritis may be "younger" trend,overweight and obesity accounted for more.The main constitution type of Hp positive patients is damp heat.The gender and age of patients with chronic gastritis in this study are related to TCM Constitution and TCM syndrome types.There is a certain correlation between TCM Constitution and TCM Syndrome Types in patients with chronic gastritis.2.After 8 weeks of treatment,most of the subjects had a significant improvement in TCM syndrome,and the overall effective rate of TCM syndrome was 87.4%.In terms of constitution and syndrome type,the curative effect of patients with damp heat syndrome is better than that of blood stasis syndrome,and the curative effect of patients with spleen stomach damp heat syndrome is better than that of patients with stomach collateral stasis syndrome.Indicating that the application of three differentiation diagnosis and treatment mode in the treatment of chronic gastritis has practical clinical effect,which provides a new and multidimensional diagnosis and treatment method for the treatment of chronic gastritis,improves the curative effect and prevents recurrence.3.After 8 weeks of treatment,most of the symptoms were improved,indicating that the application of three differentiation diagnosis and treatment model in the treatment of chronic gastritis can improve some symptoms of patients.
