| Objective: The purpose of this study is to investigate the prognostic factors of synchronous multiple primary lung cancer after surgical treatment,through analyzing the clinical data,and provide supports for the management of s MPLC.Methods: From January 2014 to January 2019,118 s MPLC patients who were diagnosed after receiving surgical treatment in our hospital were retrospectively reviewed.The binary logistic regression model was used to to assess the independent prognostic factors.Results: There were 265 primary tumor lesions that the mainly pathological type is adenocarcinoma(96.2%,255/265).Most patients had double primary lung cancer(81.4 %,96/118).The 2-year disease-free survival(DFS)and overall survival(OS)rates of this group s MPLC patients were 80.5% and 95.8%,respectively.Smoking history(P=0.005),the solid diameter of the maximum lesion(P=0.015),the diameter of the maximum lesion(P=0.015),pleural indentation sign(P= 0.037)and poor differentiation(P= 0.023)were independent risk factors for poor prognosis of s MPLC.Compared with unilateral operation group,there was no significant difference in the occurrence of severe postoperative complications in bilateral operation group,and the difference of operation time,blood loss,tube-carrying time and postoperative hospitalization time were statistically significant.Conclusion: s MPLC can achieve a favorable prognosis with early surgical treatment.Smoking history,the solid diameter of the maximum lesion,the diameter of the maximum lesion,pleural indentation sign and poor differentiation are two important prognostic predictors.There is no clinical significance in the statistical difference of bilateral operation group and unilateral operation group.For bilateral s MPLC patients,it is suggested that the simultaneous bilateral operation treatment can be selected if their cardiopulmonary function is good. |
